JAW CRUSHER WAS SHUT DOWN FOR REPAIRS I.NAME WAS CHANGING RUBBER SUPPORTS ON GRIZZLY HE SLIPPED OFF THE WORK PLATFORM AND FELL INTO THE THROAT OF THE JAW CRUSHER I.NAME WEIGHS BETWEEN 350 AND 400 LBS HE BECAME WEDGED IN THE THROAT OF THE CRUSHER WE WERE UNABLE TO LIFT HIM OUT SO WE CALLED THE FIRE DEPT THEY REMOVED HIM AND AS A PRECAUTION HE WAS TAKEN BY AMBULANCE TO ST ROSE HOSPI

HE WAS STANDING ON EDGE OF BASNK WITH RIGHT ARM EXTENDED GUIDING CONVEYOR WHICH WAS BEING SET IN PLACE. BANK GAVE WAY AND EMPLOYEE FELL, INJURING HIS RIGHT SHOLDER. HE CONTINUED WORKING HIS R UGLAR SHIFT. ON WEDN. JULY 13, 1983 HE COMPLAINED OF HIS SHOULDER HURTING AND WAS SENT FOR MEDICAL TREATMENT.
