While pre-shifting the middle deck screen media in secondary station, the associate rolled the chute forward to access the middle deck. EE proceeded to climb while grabbing the side of the screen stepping on metal plate attached to the side of the screen. While doing so EE lost EE's grip/footing falling backwards and landing on angle where the roll away chute rides on rails.

Showing all 49EE was assisting in unloading some equipment. When climbing down from the truck, EE stepped on the lift axle, the tire rolled causing EE to lose balance and fall 3' to the ground. This fall caused knee injury.
NO ISSUES UNTIL 5/26/23-SURGERY FOR ROTATOR CUFF**LOST TIME** Injury occurred after tripping over a timber while walking backwards when moving a ladder.
When EE finished cleaning out loader bucket, EE began turning while pulling the water hose. As EE turned, EE stepped/tripped over an area of uneven ground. Ground conditions had been affected from heavy rain over the past few days. Falling to the ground, EE rolled an ankle and broke it.
EE was washing out the material in the bottom of a load out bin. EE slipped on some loose material causing EE to fall onto a shelf.
Operator was climbing the ladder on the offside of the 980 loader to fill the auto greaser. When operator got to the last step right knee gave out and operator fell back to operator's right approximately 5' 4" to the ground.
The area near the primary had the snow plowed off of it the day before in preparation for the work to be performed. Overnight this area had frozen. EE got out of the truck & walked to the back where EE got salt to spread on the walkway. While spreading salt EE slipped on the ice and fell hurting EE's right leg and knee.
BECAME REPORTABLE ON 3/15/2022 AFTER EMPLOYEE HAD TO HAVE SURGERY & LOST TIME. While stepping down scale office steps, slipped on a patch of ice & fell causing injury to right shoulder.
Employee was climbing down from a D8 bulldozer, was standing on the tracks when foot slipped in the mud and employee fell off the track. Employee's wrist was lacerated by rock on the ground.
EE was dismounting EE's rock breaker when EE slipped off the track and landed primarily on head and neck.
Employee slipped, fell to knees.
While delivering material I realized that my truck was having issues. I called towing company who came & loaded the truck. Then took the truck to the quarry to unload the material. After unloading the material, I proceeded to step out of the truck only to discover that the wrecker had raised the truck further from the ground causing me to fall & injure myself.
While climbing the ladder to tie it off to the gutter, the ladder slipped out from under ee and the employee fell off.
At the end of ee shift, EE was exiting the haul truck when ee felt a pain in back.
Large rocks on the ground ee rolled ankle fell to the ground breaking leg.
While exiting the haul truck, associate slipped off the step & twisted right ankle.
WHILE CLIMBING DOWN A 980 YARD LOADER, ASSOCIATE SLIPPED ON OR MISSED THE LAST STEP. THIS CAUSED EE TO TWIST KNEE WHEN EE LANDED.
While walking out of the control house, down steps, he felt/heard a pop from behind his right knee.
1 injury; Employee was working at ground level installing an electrical "can" when he slipped on a rock, falling back. He tried to prevent impact of the fall by placing his wrist behind him. He landed on his wrist and broke it.
After checking screens, began to step down on what appeared to be snow, yet discovered there was ice under snow. As he slipped, one leg went out one way, while the other bent awkwardly underneath him.
Associate was exiting the primary crusher area after replacing toggle plate and seat. While crossing over an I-beam, he tripped and fell fracturing his wrist.

EE exited the loader facing forward. His tripped over the door jam and fell forward, catching himself with his arms. He did not go to the doctor until 07/12/2012.
While walking the associate stepped in the area where Conveyor catwalk and the stairway to Conveyor and the Crusher Platform meet. This is an area with a gap where two pieces of Grip Strut are side by side with one at a downward angle and the upward stairway. EE said that he apparently had stepped the area and heard his right knee pop.
Associate was working in crusher, ladder was let go by helper and ladder slid into crusher. Associate fell, hitting his head on rocks and damaging his arm.

Associate was climbing onto a 980g loader. He was climbing using both hands but was wearing cotton gloves which are very slippery on smooth metal. He was on the second step when his hand slipped and he fell backward approximately 36 inches to the concrete floor landing on his bottom.
Associate crawled across the frame/motor portion of the plant to access the stop switch. He slipped and cut his finger on the metal cover of the hydraulic tank and hoses.
Associate stepped on an uneven area of the ground and fell; injuring his leg.
EE was standing on bed of welding truck w/ 3 screens in front of him, leaning on welder. He pushed 1st screen onto EE's forks & screen caught his glove, pulling him off truck. He then fell to the ground and during the fall, the sharp edges of screen cut his arm, chin & neck. The arm injury was severe, with his skin being peeled back exposing muscle, veins and possibly tendons.
Associate tripped on a roll a way chute track upon exiting the tertiary bin. He then walked to electric house to call for help. Associate hurt his lower back when he tripped.
Associate was stepping down from drill carrier cab onto the ground. He landed hard on his right heel. Experienced some discomfort at that time, but it did not bother him the rest of the day. The next day his heel was swollen and painful and he went to the doctor.
Assoc was standing on back edge of welding truck platform throwing scrap metal and conveyor belts into the dumpster, bent to pick up a 3' x 42" piece of conveyor, lost balance and fell backwards breaking wrist
Assoc. dismounting excavator tracks to retrieve tools, using three-points of contact. Stepped to ground with left leg, felt knee pop.
THERE WAS AN UNGUARDED OPENING NEAR WHERE THE BASE OF A LADDER WAS SET UP. THE INJURED EMPLOYEE CLIMBED THE LADDER BEFORE HIS CO-WORKER CAME BACK TO THE WORK LOCATION FROM LUNCH. THEY HAD BEEN HOLDING THE LADDER FOR EACH OTHER. AS HE CLIMBED THE LADDER, THE BOTTOM SLIPPED & FELL THROUGH THE HOLE & HE FELL THROUGH THE HOLE TO A PLATFORM 12' BELOW.
Associate is Quality Control lab technician, he was carrying a bucket out the lab door to empty it, stepped down and his right knee twisted and popped.

A SECTION OF CATWALK FLOOR GRATING HAD NOT BEEN INSTALLED WHERE TWO SECTIONS OF CONVEYOR HAD JOINED TOGETHER. EE HAD FAILED TO PUT HIS SAFETYHARNESS ON AFTER HE TOOK A BREAK. HE WAS INSTALL ING HANDRAILS ON THE CATWALK IN THE AREA OF THE OPENING AND FELL THROUGH THE HOLE.
EE WAS GETTING INTO CAB OF EXCAVATOR, UTILIZING THREE POINT CONTACT WHEN HIS LEFT FOOT SLIPPED CAUSING HIM TO FALL TO THE GROUND.
EE SLIPPED ON FROZEN GROUND.
EE CLIMBED UPON 769C HAUL TRUCK. HE WENT TOWARDS DOOR AND SLIPPED ON ICE ON THE CATWALK.
