OPERATOR FELL A8 HE STEPPED ACROSS TWO HORIZONTAL PIPES ABOUT 1.5 FT OFF THE GROUND. BECAUSE THE ACTIONS OF THE OPERATOR WERE NOT AS DELIBERATE AS THEY SHOULD HAVE BEEN, HE NEEDLESSLY STEPPED ONTO AN UNEVEN3SURFACE ON THE OTHER SIDE OF THE PIPES. CONSEQUENTLY, THE OPERATOR FELL AND TWISTED HIS KNEE CAUSING A PARTIAL TEAR TO THE LIGAMENT IN HIS RIGHT KNEE.
