Miner was walking out of a doorway carrying a part the EE was going to do repairs on, when EE slipped on some ice. Slipping on the ice caused EE to fall to the ground and drop the part EE was carrying on EE's right ring finger.

Coming down stairs after checking HiCal system and slipped on ice on the stairs from freezing rain. Employee caught himself with his arm and strained the arm and rotator cuff.

EMPLOYEE STATED THAT WHILE HE WAS SHOVELING MATERIAL HE SLIPPED AND FELL CATCHING HIMSELF ON THE EDGE OF THE HOPPER.

EE WAS PULLING CONVEYOR BELT TOGETHER WHEN HE SLIPPED ON WET SURFACE DUE TO RAIN AND MUD AND FELL AGAINST ROLLERS ON CONVEYOR.

WHILE CLEANING OUT PLUGGED CONE CRUSHER EE HOPPED DOWN OFF CRUSHER APPROX 3' AND TWISTED HIS LEFT ANKLE.
EE ADVISED SUPERVISOR ON 4/24/00 THAT AS A RESULT OF TRIPPING ON A ROCK 3 WKS AGO HE SUSTAINED A TEAR TYPE INJURY TO RIGHT KNEE
