Miner was exiting a front end loader when EE's boot got caught on the tread of the access step resulting in the miner falling approximately 6 feet landing on their left side. Miner sustained injury to the left hip & knee, head and neck.

Showing all 4EE was dismounting loader, stepped on bottom step of loader and slipped off twisting knee. EE states the ladder was slippery and frosted. Claims a little bit of pain but it went away. EE said he noticed swelling Friday night and was painful and could not get in to see his doctor until Monday morning.
Fell while fueling loader, claimed injury to ribs, saw Dr on 5-7-04, returned to work 5-10-04, worked no problem until 5-19-04, went to Dr again, and off until 5-20-04.
