Employee was walking when EE stepped into a small hole/impression in the ground. EE's right hip shift unexpectedly causing discomfort. On Monday, while on shift, EE notified the supervisor, for the first time, that EE was in pain, then EE was taken to Concentra MC. Diagnosis Sprain of Right Hip. released with restrictions. We will provide work under those restrictions.
When the employee attempted to mount a dozer, EE slipped and fell.
As EE was dismounting a 992 pit loader EE missed the last rung of the ladder, causing EE to put all EE's body weight on EE's arm, causing a sprain to EE's left wrist.

EE complained of neck pain, but refused medical attention, was given first aid. EE told supervisor EE would notify supervisor in the morning if the pain persisted.
EE slipped into a gator hole which was covered by vegetation while walking during the performance survey duties, causing knee to impact the ground and/or twist.

EE WAS DISMOUNTING THE PAYSTAR FUEL TRUCK WHEN HIS FOOT MISSED THE STEP. HE FELL TO THE GROUND INJURING HIS HIP/PELVIC AREA.
EE SLIPPED OFF BACKHOE STEP & FELL BACKWARD HITTING HIS ELBOW ON A PIN.
LEFT FOOT SLIPPED ON LADDER CAUSING BODY WEIGHT TO SHIFT TO RIGHT LEG. THIS CAUSED INJURY TO RIGHT KNEE.
