EMPLOYEE HAD STEPPED UP ONTO 6" CHANNEL TO EXBAND HIS REACH TO GRAB SOMETHHING. THE CHANNEL WAS WET & MUDDLY CAUSING HIM TO SLIP THUS TWISTING HIS WRIST.

EMPLOYEE WAS CLEANING OFF WALKWAY. SLIPPED ON ROCK FELL AND BUMPED LEFT KNEE. EMPLOYEE COMPLAINED OF SORE KNEE THE FOLLOWING DAY. REPORTED ACCIDENT. SENT TO MEDICAL CLINIC FOR PRECAUTIONARY X- RAY WHICH WAS NEGATIVE. EMPLOYEE RETURNED TO WORK, NO FUTHER COMPLICATIONS.
