EE WAS EXITING THE KILN AT THE END OF HER SHIFT. A FORKLIFT WAS COMING INTO THE KILN AND HAD STOPPED TO LET HER GO BY. AS SHE WAS WALKING AROUND, THERE WAS DUST COVERING THE SURFACE OF THE KILN. SHE SLIPPED IN THIS, RESULTING IN HER FRACTURING HER ANKLE.
EE was stepping from the driver door of a vacuum truck to the pier at the finish silos. He missed solid contact with the pier and twisted his right knee. He reported the incident on 3/1/05 but declined medical attention until 3/11/05

EE SLIPPED & FELL ON RIGHT BUTTOCK NOTED IN MINOR INJURY LOG ON 8-29. EE STATED HIS BACK WAS HURTING FROM FALL. TRANSPORTED TO DR. RETURNED TO REGULAR WORK WITH FULL RELEASE. AFTER FOLLOWUP VI SITS STILL COMPLAINING ABOUT BACK. DR. PUT EE ON MODIFIED DUTY ON 9-17-01.
PERSON DROVE BOBCAT UNDER 71BM01 TARROR MILL TO LOAD BUCKET WISH CLEANUP DUST PERSON SHUT OFF BOBCAT WITH BUCKET ON GROUND, HE THEN LIFTED THE SAFETY ARM AND STARZED TO GET OUT OF CAB. HE STEP PED ON TO THE SUPPORT ARM FOR THE BUCKET AND STOOD UP. WHEN HE STOOD UP A BOLT SECURING AN ACCESS DOOR ONT HE MILL STRUCK PERSON ON HARD WAY KNOCKING HIM TO THE GROUND.

EMPLOYEE WAS CLIMBING OUT OF SERVICE TRUCK, STEPPED ON LOOSE MATERIAL INJURING RIGHT ANKLE.
EMPLOYEE WAS WALKING TO THE JOB SITE, STEPPED ON MARBLE SIZE CLINKERS, SLIPPED AND BROKE HIS LEFT ANKLE.
