EE slipped and fell and cut EE's right forearm. It required 6 stitches and EE was placed on modified duty.
On final day of EE's job, employee was descending a 15' fixed ladder with a cage when EE's foot slipped and EE fell approximately 10'. Employee broke EE's left fibula.

Employee was checking the position of 41TV12 NOTAM and walked toward 41FG01 NOTAM. There was a rake laying on the catwalk, employee tripped over the rake and fell down.
Delivery truck driver driving for Kim Trucking, Victorville, CA was delivering guard rail material. He climbed onto the rear of his truck to remove edge protectors from the load when he fell backwards approximately 5 feet. He landed on the paved surface breaking his RT arm.
Employee was using a chipping hammer to clean a screw conveyor. He was working overhead and slipped on some loose material causing him to twist and sprain the lower back.
Employee was adjusting the tires on the clinker reclaimer. As he walked around the reclaimer employee slipped on the clinker and twisted the left ankle causing a sprain.
Employees instructed to use JLG lift to install and tighten bolts on expansion joint. Employee untied safety harness, climbed out of JLG basket & climbed over onto an unsecure platform to allegedly look for an air hose. Employee did not tie off and fell through a floor opening. He fell approx 3-5 feet onto "X" bracing and slid down to top of classifier another 4-5 feet.
Employee passed out in the change room. As he fell to the ground he hit his head on the bench and strained his back.

A customer cement truck driver working for State Ready Mix Company slipped while opening a trailer hatch in preparation for loading. He fell against a guardrail, striking his R. lower back. Resulted in 2 fractured ribs. This is not a mine employee.
EE WAS EXITING THE KILN AT THE END OF HER SHIFT. A FORKLIFT WAS COMING INTO THE KILN AND HAD STOPPED TO LET HER GO BY. AS SHE WAS WALKING AROUND, THERE WAS DUST COVERING THE SURFACE OF THE KILN. SHE SLIPPED IN THIS, RESULTING IN HER FRACTURING HER ANKLE.
EE was stepping from the driver door of a vacuum truck to the pier at the finish silos. He missed solid contact with the pier and twisted his right knee. He reported the incident on 3/1/05 but declined medical attention until 3/11/05
EE was carying a load of materials down a short flight of steps and slipped falling forward. EE reported incident on 1/5/05 and declined medical treatment. On 3/24/05 EE was evaluated by a physician but no treatment was prescribed and no restriction were imposed. On 4/19/05, NCC learned that physical therapy was ordered for L. knee & lumbar strain
Employee attempted to lift vacuum hose while cleaning in the vicinity of 62BC10 conveyor inside the clinker building. Employee was standing on a sloped surface with some loose clinker on the surface. Employee lost balance and fell (same level) on his back.
Employee was assisting to unload damaged conveyor belt in scrap yard. He stepped on the belt rubber and slipped on loose clinker falling (same level) on his back.
Employee was standing on an angled support with the right knee braced against a concrete peir at ground level to tighten a fan bearing bolt. His left foot slipped on the angle, shifting stress to his right knee
AFTER ADJUSTING THE COOLING WATER VALVE ON A LUBRICATING UNTI 71LU14 FOR FINISH MILL #3 71BM03, THE EE STEPPED ON THE EDGE OF A RAISED FOOTING OF THE STRUCTURE SUPPORTING THE MILL EQUIPMENT AN D TWISTED HIS LEFT ANKLE. THE INCIDENT RESULTED IN A LEFT ANKLE SPRAIN.

EMPLOYEE WAS CLIMBING DOWN SPIRAL STARIS AT TRUCK LOAD OUT AND SLIPPED AND SLID DOWN STAIRS.
EMPLOYEE WAS CLIMBING UP A STAIRWAY IN ELECTRIC SHOP - CAUGHT FOOT ON STEP AND TWISTED HIS KNEE.
SILO #4 PLUGGED. EMPLOYEE WAS STANDING ON HAND RAIL TRYING TO UNPLUG THROUGH THE CLAY BAILEY. WHEN EMPLOYEE STARTED TO CLIMB DOWN, SLIPPED OFF OF RAIL AND FELL ONTO THE PLATFORM STRIKING KNEE.
EE SLIPPED & FELL ON RIGHT BUTTOCK NOTED IN MINOR INJURY LOG ON 8-29. EE STATED HIS BACK WAS HURTING FROM FALL. TRANSPORTED TO DR. RETURNED TO REGULAR WORK WITH FULL RELEASE. AFTER FOLLOWUP VI SITS STILL COMPLAINING ABOUT BACK. DR. PUT EE ON MODIFIED DUTY ON 9-17-01.
PERSON DROVE BOBCAT UNDER 71BM01 TARROR MILL TO LOAD BUCKET WISH CLEANUP DUST PERSON SHUT OFF BOBCAT WITH BUCKET ON GROUND, HE THEN LIFTED THE SAFETY ARM AND STARZED TO GET OUT OF CAB. HE STEP PED ON TO THE SUPPORT ARM FOR THE BUCKET AND STOOD UP. WHEN HE STOOD UP A BOLT SECURING AN ACCESS DOOR ONT HE MILL STRUCK PERSON ON HARD WAY KNOCKING HIM TO THE GROUND.
CLIMBING LADDER-FELL
EMPLOYEE WAS CLIMBING OUT OF SERVICE TRUCK, STEPPED ON LOOSE MATERIAL INJURING RIGHT ANKLE.
EMPLOYEE WAS WALKING TO THE JOB SITE, STEPPED ON MARBLE SIZE CLINKERS, SLIPPED AND BROKE HIS LEFT ANKLE.
