CUSTOMER HAD PULLED ONTO SCALES WITH SEMI-TRACTOR AND TRAILER. HE STEPPED OUT OF TRUCK ONTO SCALES AND THEN FELL BACKWARDS ONTO THE GROUND, WHICH IS A DISTANCE OF 42". THERE WERE 2" X 4" BOARD S IN FROM ALIGNMENT FOR CEMENT. HE LANDED ON ONE OF THOSE.

EMPL WAS CUTTING TROUGH THAT WAS APPROX 10' IN AIR AFTER CUTTING TROUGH HE BEGAN KICKING ON IT TO KNOCK IT LOOSE WHEN IT BROKE LOOSE HE FELL WITH IT LANDING ON BAR THAT WENT ACROSS TROUGH.
