EE started pump at remote quarry location, turned, homeless man (been there before) was right there. Man grabbed/slammed EE to ground, ran away. EE's wrist injured as EE tried to break fall. Dr diagnosed as left wrist pain with restrictions still allowing EE to do normal duties. Follow up x-ray was taken, revealed small fracture/bone chip in wrist. The police were called.

While fueling a loader, staged away from the quarry, fueler slipped on ladder attached to loader, falling to ground, injuring elbow. Weather conditions were wet/rainy and fueler's gloves were wet, causing the fueler to lose his grip on the ladder.

Stepped out of truck, twisted right knee. Was originally diagnosed as a sprain. Continued working. 4/19/13 Dr requested MRI. On 4/24/13 patient met with Dr about MRI and it was determined that he has a torn meniscus. On 4/25/13 patient is put on lost time and surgery is pending.

Employee was in process of removing a steel hopper side. He was cutting what he thought was 1 of last 4 bolts securing panel when it was actually the last bolt. He may have been leaning on panel when it gave way creating opening causing him to lose balance & fall thru opening 21ft to ground. He was wearing fall protection but was not tied off to the provided life line.

EE reported hurt his right knee 10/30/09 approx 12:00PM. EE reports he was walking across rocky ground , rock broke & he fell backwards. EE said when he fell he twisted right knee, it hurt at the time but didn't think much of it. 11/9/09 went to dr for check, put him in knee sleeve. 11/16/09 dr gave cortisone shot. No lost time, is working.

EE WAS WELDING ON THE PRIMARY CRUSHER AND WAS TURNING THE DRUM WITH HIS FEET. HIS FOOT SLIPPED OFF AND STRAINED HIS NECK AND SHOULDER. HE CONTINUED TO WORK TO 7-8-04. THIS WAS HIS FIRST DAY OFF WORK. HE WENT TO THE DR. FOR TEST.

EE WAS PULLING ON THE ROPE FOR HIS TARP ON THE TRUCK #119. THE ROPE BROKE AND HE FELL BACKWARDS AND PUT HIS LEFT HAND OUT TO CONTROL FALL. HIS ARM ABSORBED ALL OF THE ENERGY. HIS LEFT ARM BROK E AT THE WRIST.
