IE was unloading boxes from the tailgate of a pickup truck and carrying them into a Connex storage container. As IE stepped up into the container, EE's foot slipped and EE's right leg came down hard onto the ground causing a sprain to EE's right knee.

During the fall, the employee attempted to regain balance by grabbing the door, which caused EE's body to shift to the right. Upon impact, EE landed on EE's right elbow and reported pain.

Plant Tech was walking a leach pad doing routine checks when they tripped on a dripper hose falling onto their knees
Employee was dismounting dozer in the wash bay. The foot on the dozer arm slipped and they fell backwards getting right leg hung up between the track frame and the dozer arm for a short time. After leg came free, employee fell to the ground. X-ray revealed a fracture at the end of the femur bone where the ligament attaches above the right knee.
Employee was walking with a short flathead shovel into the tool room at the process plant when EE tripped over a cushion mat on the floor, while trying to catch themselves the shovel handle made contact to the front teeth causing the left one to chip in half.

After priming a blasthole, the ee stepped back to kick some drill cuttings away to bury the down line. When he stepped back he fell into a blasthole that had been re-drilled. He experienced pain in right knee but did not know it was a serious injury.

WHILE PRESSING A BEARING INTO THE BOOM ON A CAT 992 LOADER, THE TOOL BROKE. THIS RESULTED IN THE HYDRAULIC HOSES HITTING EE CAUSING LOSS OF BALANCE AND HE FELL ABOUT 9.5 FT. HE HAS A BROKEN WR IST ON THE LEFT ARM AND PULLED MUSCLES IN HIS LEFT SHOULDER. HE ALSO HAS A CUT ON THE FOREHEAD WHICH TOOK SEVERAL STITCHES.
