Employee stepped incorrectly and sprained/strained their right ankle.

Employee advised stepped wrong and twisted knee while working cleaning a car.

Employee tripped demounting a platform in a packaging area hurting their knee. Employee finished working the shift and returned to work the following day to complete their weekly work schedule. On Nov. 7th employee sent for medical evaluation due to pain. Employee removed from work due to fractured left patella.

Employee was traveling between the maintenance shop and the "Bubble", to shut off cutting torch bottles. Employee states they "stepped wrong" and rolled their left ankle.

Employee lost footing, slipped or fell into belt manlift, internal injuries to upper torso.

Delivery driver from Rhinehart Oil preparing to pump oil from truck tank to Dicaperl tank. Truck bed has raised seam which cause delivery driver to trip and hit head on a rail.

While opening doors on top of train car, employee dropped hammer in compartment. Went in train car to get it, slipped and hit right elbow on middle beam.

Missed step tried to catch self and pulled EE's leg.

Employee turned EE's ankle walking to a fire drill.

Slipped injured knee and ankle/foot

EE was on top of railroad car to move storm lids from opening on the car to prepare to move the car to the next loading position. He stated that he was moving the lid and slipped falling into the hopper car partially loaded. he did not have his safety belt on at the time of the incident.

Employee was fueling loader off of fuel/lube truck. He stepped onto bed of lube truck to shut off fuel switch when he tripped over a pipe on the truck bed causing him to fall.

EMPLOYEE WAS COMING BACK DOWN STEEL LADDER AFTER CLEARING #6 ORE TRANSFER SCREW CONVEYOR ON DECK NEXT TO #6 FURNACE AND MISSED A STEP AND FELL. INJURIES TO LEFT SHOULDER, LEFT LOWER SIDE/BACK AND HEAD AND ELBOW.

EE WAS STANDING ON A LADDER WHICH CAME OUT FROM UNDERNEATH THE EE CAUSING EE TO FALL. EE WAS GETTING AN ORE SAMPLE FROM THE CHUTE ON SCREEN DECK.

TWISTED ANKLE WHILE STEPPING TO GROUND FROM RAIL CAR AFTER LOCKING BRAKE ON RAILCAR.

HE WAS DESCENDING STAIRS FROM SCREEN DECK. FOOT SLIPPED ON STEP & SKIPPED A FEW. IT WAS RAINING.

SLIPPED ON SPILL LEFT ON FLOOR AND ALLEGED SPRAIN OF LOWER BACK.

EMPLOYEE WAS REPLACING A SPRING ON A COMPRESSOR WHEN HE HIT HIS RIGHT KNEE ON THE COMPRESSOR FRAME.

EMPLOYEE WAS WALKING UP STAIRS WHEN HE STEPPED ON A PIECE OF ICE AND THE TOP OF A STEP, SPRAINING HIS RIGHT ANKLE.

EMPLOYEE WAS CHECKING RADIATOR WATER LEVEL ON D8CAT DOZER, HE STEPPED OFF, SLIPPED AND GRABBED HAND RAIL WHEN HE FELT A PAIN IN HIS LOWER BACK.

EMPLOYEE STEPPED ON 2 WELDING RODS, HIS FOOT ROLLED OUT FROM UNDER HIM, CAUSING HIM TO FALL.

EMPLOYEE WAS REPLACING SIMPLICITY SCREEN ON SCREEN DECK. IN REMOVING DUST COVERS FROM SCREEN, EMPLOYEE SLIPPED ON SECOND RUNG OF LADDER CAUSING HIS KNEE TO EXTEND.

EE PROVOKED ANOTHER EE WHO PUNCHED HIM CAUSING HIM TO FALL ON HIS ELBOW AND BACK.

EE WAS HELPING CARRY A METAL PLATE WHEN HE STEPPED IN A HOLE IN THE FLOOR & HURT HIS LOWER BACK.
