THE EMPLOYEE HAD FINISHED CHANGING SCREENS WHEN HE STEPPED DOWN ON THE CATWALK APPROX. 2 & 1/2' BELOW. WHEN HIS FOOT LANDED, THE INJURY OCCURED.
THEY RAN THE WATER HOSE UP THE LADDER, WHEN EE CAME DOWN, HIS FOOT SLIPPED ON THE HOSE. HE CAUGHT HIMSELF WITH HIS RIGHT ARM, INJURING IT.
IT WAS FOGGY MORNING WITH HEAVY DEW, THE LADDER WAS WET FROM THE DEW-HE WAS CLIMBING THE LADDER TO GET INTO THE MACHINE-HE SLIPPED AND FELL BACKWARDS ONTO THE GROUND INJURING HIS BACK. PULLED MUSCLE.
