Showing 50 of 70EE was walking down the stairs from the K3/K4 burner floor towards the K4 coal mill while carrying samples. EE stepped down from the last step in the stairs, rolled EE's ankle and fell to the ground.
IE clocked out went to EE's car and changed out of work boots and put on slides, went to the back of EE's car, stepped off asphalt into muddy area and fell. Broke 2 bones in EE's ankle.
The employee was cleaning the roof on the top lime silo with a pressure washer. After EE finished part of it, EE went downstairs between the 5th and 4th floor and slipped on a step. EE was holding onto the handrail with right hand.
EE stepping down off a ladder and missed a rung causing EE to strain shoulder.
Employee was walking between rows of drilled pattern on a 21 degree slope (approximate) when EE stepped on a loose stone with EE's right foot causing EE to fall to the ground resulting in a right ankle sprain and right knee contusion.

After clearing the chute, EE was coming down the ladder and stepped off the last rung onto the small concrete. On EE's next step EE missed the concrete landing and fell 13" to the concrete level below and hit left shoulder. It was raining very hard at the time of the accident.
Employee was walking toward equipment. Stepped on an object, possible rock, fell to the ground twisting left ankle.
