A loader operator stopped the machine. When exiting the loader, turned to face the cab of the machine to begin climbing down the ladder not establishing 3 points of contact, losing its balance and falling to the ground. Management immediately executed the emergency protocols. No significant injuries were observed at the time of the fall.

Employee was working on a conveyor installing a belt saddle. As employee walked from the South side to the North side of the conveyor to deliver tools, the employee slipped in mud while crossing under the conveyor.
Employee was closing bottom dump rail car hopper doors when EE slipped and broke ankle.
Attempting to install bit into hydraulic hammer. EE was standing atop a rock pile when the rock the left foot was atop rolled causing EE to fall. The employee attempted to catch self causing injury to left arm/wrist.

EE WAS STANDING ON THE WATER TRUCK PLATFORM PUTTING ON A SWEAT SHIRT OVER HIS HEAD. STEPPED OFF THE EDGE OF PLATFORM AND FELL APPROXIMATELY 8 FT TO THE GROUND.
