Employee was cleaning hardened material from around piping when released material and water caused EE to lose footing and fall into the water. Employee struck the left side of EE's body, head, & left leg/knee on piping & was briefly submerged before being assisted by a coworker. Initial treatment provided by site nurse. Employee was later removed from site pending medical release.

Showing 50 of 64While tied off to angle iron, employee was climbing on top of shaker to take measurements. As employee took a step, went to grab hold of angle iron for balance. Potash buildup loosened and slipped. Employee lost balance and fell to floor. Resulted in broken left forearm.
While EE was in the process of fueling an underground diesel mantrip, EE had stepped off of the vehicle, lost balance causing them to trip and injure right ankle. EE was taken to an offsite clinic for treatment & x-ray. The injury has been determined to be a severe sprain/strain to Right Ankle. Restrictions as follows: No walking on uneven surfaces and seated work only.
While EE was walking on the backside of the belt storage unit near 10 LML, EE walked RT foot into a unseen hole due to excessive fine dust. EE continued walking stepping with the LT Foot into another unseen hole causing the EE to feel pain in the lower back restrictions: no lifting, carrying over 20 lbs, no pushing/pulling over 20 lbs, no climbing, no stooping, no crouching.
EE was walking going to step over a raised area right foot leading. EE stepped over in a way that when EE's right foot hit the floor with all EE's weight, EE stated that EE heard and felt a crack and pop noise from EE's knee. Majority of pain located on back side of knee, both sides. After MRI confirmation on 10/14/2024. EE has a Torn Meniscus No Restrictions.
In Area 22 operator was walking back from mining face when EE tripped over miner cable falling backwards. Operator stuck arm out to brace the fall causing injury to right elbow. EE was diagnosed with a hairline radial head fracture to the right arm and a dislocated right elbow. EE was given prescription narcotics for pain and general anesthesia to reduce the elbow back in place.
Employee was performing daily rounds, checking for clean-up. Employee walked down to a lower deck and rolled ankle on the last step and fell on to the graded deck.
Employee was exiting a vehicle and felt pain in left knee. Employee went to the ER on 1/7/22 related to increased swelling to the left knee. Employee was given a prescription med and placed on restrictions.
The incident occurred in a parking area. The walking surface is paved. At the time of this incident the walking surface was dry, flat, and well lit. There were no safety hazards in the area. The employee's knee gave out with no assistance from any onsite hazards. The employee fell on left knee and was later discovered that left tibia had fractured and required surgery.
Going down stairs, slipped and fell forward, hit chest on a concrete foundation. Complained of chest pain and shortness of breath. Taken to ER and given a prescription for pain and released without restrictions.
Emp. had just spotted a string of rail cars at Storage & Loading, #4 Load out. Emp. exited the shuttle wagon to decouple the rail car from the shuttle wagon. The emp climbed between the rail car & shuttle wagon and fell from an elevated height of 42 inches. Emp has fractured ribs, fractured vertebrae, lacerated lung and adrenal gland. They was admitted to the hospital.
Employee was roof bolting and stepped in a slightly uneven cut and hyper extended left knee. Emp. went to Dr. no restrictions or meds. On 5/17/2017 emp. had surgery for torn Meniscus. Emp on restrictions and pain meds as of 5/18/2017.
Emp was preparing to wash out the bed of the battery car. Emp was standing on top of the battery car and fell 62" to the ground. The employee struck right eye area on an orange traffic cone lying on its side. Emp taken to ER, has fx to eye and had stitches. No restrictions.
Employee tripped on an elevated piece of the roadway. The asphalt had recently been removed due to trenching work that was done. Back fill had eroded away due to high winds causing an approximate 2-inch elevation change in the walking surface. Taken to ER, has fracture to right shoulder. Given prescription meds and placed on restrictions. Surgery pending.
Emp stepped off of equip & missed a step. He felt pain to his R ankle & then fell to the ground. Emp reported incident to his team leader and cont working, but started to limp by the end of the shift. When emp took off his boot at home his ankle began to swell so he went to the ER. X-ray was taken & showed a stress fracture. Emp placed on restrictions.
Emp. was using a ladder to access the North Secondary Screen discharge chutes. Emp. fell off the ladder, striking his head on equipment causing a laceration to the back of his head. Emp. was taken to Dr. and required 2 sutures to back of his head. No restrictions noted.
During rounds at the crusher at the 1026 belt, the employee's ankle rolled while walking on the cat walk. There was nothing in the walkway and no other hazards present. Taken to company Dr on 8/26/13 and placed on restrictions.
Emp. slipped or tripped while pulling an underground cable. He fell into a trough on his right side hitting his head, hip, and knee and said he felt like he twisted his ankle. Emp. taken to Dr. placed on restrictions, able to work job with restrictions. Confirmed torn meniscus on 2/10/12 and reported to MSHA.
Emp. parked ram car and exited vehicle. He stepped off the car onto some loose muck, his right foot twisted and he rolled his right ankle. He fell to the ground. He was taken to ER and placed on restrictions and given a prescription medication.
Emp. was walking with a fluorescent bulb. She set the bulb down and walked down one step she attempted to pick up the bulb and finish the next two stairs; however fell and stated she could not remember what happened. Emp. taken to ER and did have a fracture to her left leg.

Emp. was coming off ladder, stepped off last rung and twisted his right knee. He was taken to the dr. and placed on restrictions. He did state he had injured this knee in the past.
Emp. was working on West Tails Screen applying silicone on a crack between the poly decks and discharge lip on screen. When he finished he turned around to climb out and twisted his right knee in the process.
Emp. was adjusting gate in storage tunnel when the clean up crew was moving the suction hose from the vacuum truck and knocked his feet from under him and he fell on his knee and shoulder. Emp. stated that it was very dusty in the tunnel and could hear the hose moving but didn't know it was that close to his feet. Taken to the doctor on 12/10/10 and placed on restrictions.
Emp. walking to the belt controller & tripped on a turnbuckle & anchor. He fell face down on the floor. Emp. denied pain at the time of injury. He contin. to work & his left knee started hurting. Emp. was off on personal leave & notified the Safety Dept. on 12/8/10 that he was still having swelling & pain to his left knee. Emp. taken to doctor & placed on restrictions on 12/9/10.
Emp. states he parked a front end loader and when stepping out from the loader, he caught his right foot in the last bottom step. Emp. fell to the ground and when he got up he felt pain in his right knee.
Emp. stepping backwards and fell into a hopper. Another emp. had removed the grating over the hopper without notifing the emp. that stepped into it.
Team leader pulling a water hose with mantrip, hose came around a mantrip & caught the right front tire, at this point water hose stretched, came loose. Hose wrapped around emp. legs, which caused emp. to fall on ground on his left side.
Employee was exiting a battery car. His right foot got stuck between the operator's seat and the side of the cab as he was stepping out of the car. He lost his balance and his weight shifted to his left leg. He states he heard his left knee pop.
Emp. had accumulation of product on the bottom of his boots, as he stepped down from bottom step of stairs he slipped and hit his elbow on the handrail of the stairs. Emp. bruised his elbow and strained shoulder.
Descending stairs on second landing from bottom twisted right ankle on uneven grating, caught self on handrail. Sent to company dr. not placed on restrictions until 01-28-2010.
EE was taking bolts out of cutting head motor. He was standing on side of pan when his right foot slipped causing his left knee to hyperextend. Complained of pain so went to company doctor on 10/20/09 and back on 10/27/09 - restrictions applied. EE back on restrictions 12/18/09 due to continuing pain. Waiting for orthopedic specialist appt for 2nd opinion.
Employee was climbing up the stairs (approx 7) and felt his knee pop, causing it to hurt. Rested the knee for a while but it continued to hurt so he went to company doctor on 11/16/09. Had Surgery on 12/29/09 and was placed on restrictions.
Emp was disconnecting motor drive on kmag thickner drive. He then turned and stepped back catching his foot on the conduit cover on the landing. The emp fell back and caught himself on the handrail but twisted his right knee. Went to company doctor and placed on restrictions and prescribed a medication.

Employee was descending stairs at #4 loadout coming down from the screw deck. He stepped on the edge of the stair with his heel causing his right knee to jar (hyperextend) causing immediate pain. He was holding onto rails (3 point contact) Placed on restrictions.
Emp. had been standing on truck bed of maintenance truck, putting a tarp on welding machine. When emp. stepped down his foot caught on the ball hitch, causing him to lose his balance and fall backwards and land on his left elbow, onto the concrete.
on 11-02-08, emp. was cleaning above recycle bin while wearing a safety harness, he slipped backwards and grabbed onto beam to avoid falling and felt pain in rt. shoulder. Took to comp. doctor, was ordered P.T and restricted duty.
Emp reported that while working overtime in granulation plant she felt tightness and tenderness develop in her left knee. Unknown cause. Emp knee swollen with pain. Iced and elevated and took ibuprofen but no relief. Went to company doctor and received anti-inflammatory and restrictive duty.
Emp. was unloading supplies into a metal storage container. The doors were secured by rope in an open postion. The wind started blowing heavy, causing the metal doors to slam shut, hitting emp. in the back and knocking him down. Went to ER for evaluation, placed on restricted duty.
While in process of stepping back into ram car, emp. slipped and fell against the cab causing him to fracture two ribs (left side). There was hydraulic oil on the floor of the ram car which caused him to slip. Employee placed on restrictions that state seated work only.
Employee slipped and fell. When falling he put his left hand out to break the fall and the fingers went through the grating. Went to company doctor, placed on restrictions, XRAY negative for fracture.
Employee got out of the ram car and was turning around to face the steps and felt pain in his right knee. Went to company doctor on 7/6/07 and was placed on restrictive duty. Employee was on days off until 7/11/07. He returned to doctor on 7/10/07 and was released to return to work on restrictive duty. Employee is able to do all job duties while following ordered restrictions
Employee had just finished making a splice at belt when he had to go to the back side of the belt to push the by pass button to turn belt on. He rushed back to the belt to spot the next splice and he tripped over the grip hoist rope causing him to fall on his arms and knees. Went to company doctor on 6/22/07 placed on restrictions. Negative x-ray.
Original date of injury is 6/11/07. Employee received treatment at the time but is now experiencing increased pain and had a knee replacement on 11/7/07 and has not returned to work at this time. Left knee injury.
4/4/07 Employee stepped off of the top of the skip and felt pain in his right ankle. Slight swelling noted - ice applied. On 6/7/07 employee back to company doctor due to swelling and pain, ordered restrictions that state: no climbing stairs, off of feet every 15 minutes out of every hour. Physical therapy ordered 3 times a week for 2 weeks.
Employee was pulling on a rope to move the arm of a pivot hoist. The rope broke and he fell backward landing on his left hand and wrist. The fall caused compression and simple fractures of the wrist.
Employee fell through grating cutting his L Leg.
Employee missed a step and landed on his right foot causing it to twist; felt sharp pain.
