An employee was standing on VIS Crusher, trying to unclog/clean unit. The employee was holding a 1-1/2" water line, awaiting it to be turned on so that they could wash the clogged VSI. When the water was turned on, the employee lost their balance and fell onto the deck below EE, approximately 4.5 feet below where EE was working.

Showing 50 of 51Operator was dismounting / stepping down from a CAT Loader and when their right ankle stepped down on the ground, the operator felt a pop and couldn't put weight on it. The loader was parked on level ground with dry conditions.
Employee was working inside the cab of a bulldozer. Rain started so the employee quickly shut the doors on the bulldozer & began to dismount using three points of contact. Employee's right foot slipped on the blade push arm, causing the employee to fall. The employee came to rest on the bulldozer push arm with legs straddling each side, causing contusion to legs & lower back.
Miner parked loader, descended ladder and tripped over a wheel chock on the ground landing on left side /arm. After subsequent evaluation the next day miner was placed on restricted work.
Employee was climbing down a ladder, believed they were at ground level, stepped back and fell aprox. 3-4 ft to the ground.
Contract employee noticed smoke coming from haul unit. EE placed unit in park and exited cab to inspect engine. After noticing a fire in the engine compartment EE jumped over the rail of the 770 haul unit and fractured left leg when landing on the ground.
Employee was climbing down the ladder of a 980K loader when the bottom rubber step gave way while performing routine maintenance. Employee fell approximately two feet to the ground striking back and left wrist.
On 6/13 a Mechanic dismounting CAT 980K Loader missed a step on ladder and while catching themselves, strained right shoulder. Employee managed to continue to perform all work duties until 7/31 when seen by a doctor and was prescribed Physical Therapy and restrictions.
Employee (Maintenance Lubricator), was walking in plant slip/trip/fall spraining right thumb.
2/6/2018, employee was walking on ground level walk way, a hose from a super sucker vacuum truck was lying across walkway. As employee attempted to step over hose EE tripped and fell. Employee injured right knee. Employee was able to continue to work regular work and shift at full time status. 7/31/2018 employee diagnosed with meniscus tear and received surgery.
On 1/11 employee slipped and strained knee. Worked two scheduled days was then off two scheduled days. When employee returned to work on 1/16 requested medical was seen and prescribed physical therapy with restrictions.
Employee was cleaning under a scalper screen when stepped wrong and sprained ankle.
While walking, ee slipped on clinker landing on left forearm & wrist.
Walking down stairs in FM-5 from second step to ground floor. Twisted right ankle, felt a crunching.
While walking on top of silos, he slipped & fell on wet flooring to same level. As he tried to break his fall by using his right arm, it took the brunt of the fall.
While securing dome lids on customer trucks, he went to close the lid on a truck. As he did the driver thought that it was ok for him to proceed. As the truck drove off from that station, the employee securing the lids fell, but was wearing fall protection.

The IP attempted to handle a 20 foot long piece of square tubing alone. The IP had the tubing on his shoulder, lost his balance, and strained his back. The IP was restricted to modified duty and issued prescriptions on 3/28. Injury was determined reportable on 3/29.
While walking on uneven ground, he rolled his right ankle.

While pushing the plywood under the belt, He lost grip on the plywood and scraped his right arm on the sheet of plywood. EE stepped on top of concrete pedestal to climb up a beam. He slipped and fell 5' and sprained his wrist.

WHILE DESCENDING FROM AN UPRIGHT LADDER, EMPLOYEE'S FOOTING SLIPPED, CAUSING EMPLOYEE TO FALL APPROXIMATELY 3 FT TO GROUND, TURNING HIS RIGHT FOOT.
EE WAS CLIMBING UP THE STAIRS/LADDER TO HIS LOADER WHEN HE SLIPPED, FALLING TO THE GROUND. HE HIT HIS HEAD ON THE CONCRETE CONTAINMENT FOR OUR FUEL STORAGE.
EE WAS FINISHED REFUELING EXCAVATOR WHEN HE FELLBREAKING HIS RT. WRIST AND BRUISING HIS RT. HIP.
EE WAS DESCENDING THE LADDER OF HIS LOADER WHEN HIS RIGHT FOOT STEPPED ON AN AIR HOSE WHICH CAUSED HIS FOOT TO ROLL. HE FELL ONTO HIS BACK.

EE WAS WALKING ON A CATWALK OF HOT MIX TANK WHEN CATWALK GAVE WAY. EE FELL 6-8' TO THE GROUND, A PORTION OF THE CATWALK STRUCK EE IN THE LOWER BACK AREA.
EE WAS CLEARING OBSTRUCTION IN CRUSHER-HE LOST HIS BALANCE AND FELL AGAINST THE CATWALK RAILING BELOW.
OPERATING DOZER EE ATTEMPTED TO EXIT DOZER AND SLIPPED, LODGING HIS LEG BETWEEN BLADE ARM AND TRACK.
EMPLOYEE WAS WALKING OVER TO HIS WATER WAGON. THE ROAD HE WAS WALKING ON HAD BEEN WET DOWN FORDUST CONTROL. HE SLIPPED AND FELL, BREAKING HIS LEFT WRIST. THE SHIFT STARTED AT 6:00AM. NORMAL PROCEDURE IS TO MEET AT THE BREAK ROOM AND BE TRANSPORTED TO EQUIPMENT IN THE TRUCK.
