INJURED WAS TAKING A WATER SAMPLE FROM PINE CREEK USING A DIPPER ATTACHED TO A ROPE. HE STATED THAT THE ROPE WAS TOO SHORT TO ALLOW HIM TO TAKE THE SAMPLE FROM BEHIND THE GUARD RAIL ON THE BRI DGE. HE STEPPED OUTSIDE THE RAIL AND SLIPPED OFF THE BRIDGE FALLING INTO THE CREEK DISLOCATING HIS RT ANKLE.

EMPLOYEE WAS WALKING DOWN A FLIGHT OF STAIRS IN THE MILL. HE MISSED A STAIR AND WHEN HE CONTACTED THE FLOOR, HE FELT A PAIN IN HIS LEFT GROIN AREA.
