Employee step down on two steps stair and twisted his ankle then fell to the ground. The ankle swelled and, so employee left to go home in Sparks Nevada. Employee went to a health clinic and the ankle is fracture. Employee has not missed any work days as he is an administrator and will be working from an office to do his work.

Showing all 7EE is a dump truck driver/owner operator working thru T407. As he was on standby work due to a blasting operation, he was staged outside the pit area and in the North/East area of the quarry. While standing outside of his truck on a pile of material, he proceeded to step off the pile and lost his footing, thereby falling onto the ground.
Injury occurred while pile butt was repairing handrail at the Quarry spring line. He cut the handrail away and lost footing resulting in a fall to the water below, where he struck a float in the water. This resulted in two broken ribs and a punctured lung. Employee will be off duty until he has rehabilitated. Lost time injury.

HE SLIPPED AND FELL WHILE WALKING ON THE LOADING DOCK. HE STRUCK HIS LEFT HAND ON A METAL GRATING CAUSING A LACERATION. PROVIDED GLOVES BUT WAS NOT WEARING THEM.
