Ore haul driver was descending the ladder on a 950GC loader after loading the truck. Driver did not have good footing on the second from last step, left foot slipped off step hitting the metal behind step and bottom step. Driver was transported to the ER and diagnosed with a strained foot.

American Gypsum Company LLC · Fall to the walkway or working surface

Employee was installing chains on his truck at the start of shift. He removed the chains from the rear of the trailer and started to carry them to the truck rear driver location. The ground in the area was icy and he slipped and fell on his back. He proceeded to put the chains on, walked away from the truck and then felt a sharp pain in his left rib.
