He was working on the conveyor adjacent to the belt and when he was pulling belt off he was walking down conveyor and missed the three step ladder that led to the flat level of the plant causing him to fall and injure himself.
Employee was walking to the truck door after putting antifreeze jugs into the bed of the truck. Overturned ankle on a clump of dirt causing left ankle sprain.
Employee was inventorying stockpiles. Walked on slope of stockpile. Left leg dropped into soft spot. Attempted to pull himself out when he felt a pop in his left lower calf. Diagnosed as muscle tear to lower left calf.
EMPLOYEE WAS EXITING A LOADER. HE STEPPED TO THE GROUND AND OVERTURNED HIS ANKLE WITH HIS KNEE HITTING A ROCK WHEN HE FELL.
EMPLOYEE WAS PARKING HIS LOADER AT THE GARAGE. WHEN HE EXITED THE LOADER HE SLIPPED AND FELL FROM THE TOP STEP OF THE LADDER.
EE HAD DUMP TRUCK PARKED UNDER LOADING BINS. SHE CLIMBED UP ON TRUCK TO SEE IF IT WAS LOADED. SHE SLIPPED ON THE LADDER AND FELL TO THE GROUND.
EE WAS WORKING IN QUARRY, TRIPPED AND FELL ON RIGHT KNEE.
