Miner was stepping down off fork truck and twisted left ankle. There was no swelling and little pain at time. Miner completed shift. Over next few days, ankle continued to bother Miner. Visited doctor on 8/22 and 8/25/2008. Off work on 8/22 only. Diagnosed as sprain.

Miner states he injured left knee while climbing stairs. Did not notify management until 02/05/2008. Injury disputed by Workman's Compensation due to pre-existing problems with knee. Claim upheld on 04/14/2008 and IMV Nevada notified on 04/21/2008. Accident Report filed 04/21/2008.

EE states, he was working on palletizer with other and slipped with 50lbs bag and his leg went one way, and he fell on left and broke his fall with knee on metal rail.

EE STATED, SHE WAS GOING TO HER CAR TO THE MILL AREA TO CLEAN, SHE WAS CARRYING SUPPLIES DOWN THE FRONT STEPS TO HER CAR WHEN SHE TWISTED HER RT. ANKLE AND FELL, BRUISING HER UPPER RT. ARM.
