Employee slipped on mud and fell (same level) hurting his right upper thigh.
Employee was on ladder propped against loader. At top of ladder went to step off onto loader and ladder slipped causing him to fall 7-0 ft to ground. Right knee injury.
He was walking across stone & slipped on mud, falling. Hit his right arm & shoulder. Went to Dr who said sprain, no recheck unless problems. He wanted to go back to Dr on 10/2/08. Had a note to return 10/9/08 for recheck. Went back to Dr 10/8/08 with lumbar strain & high restrictions with no walking. Out as of 10/9/08.
EE fell off ledge, 12' to next ledge, catching himself on his knees and hands.
