Employee was standing on the third run of a ladder (3') trying to take off a nut with a ratchet style wrench. The wrench slipped out of the employee's hand causing him to lose his balance and fall on his right side. The employee tried to break his fall with his right hand causing a facture to the wrist.

The EE was taking out the trash from the scale office. While exiting the building, she rolled her ankle on the outside landing. Her right ankle had small bone fragment found on exray.

Injured party was attempting to clean aggregate buildup from side walls. of bin: stepped into bin, and onto buildup: material broke free causing the injured to slide to the bottom of bin. Remainder of material followed, burying injured to mid thigh area.

EE WAS UP ON A PORTABLE LADDER AGAINST THE LIME SURGE BIN CLEANING THE LEFT OVER LIME BUILD UP OF THE SIDES. EE SLIPPED OFF LADDER AND CAUGHT EE WITH RIGHT SHOULDER CAUSING A MILD STRAIN.

EMPLOYEE WAS WALKING FAST TO SHUT OFF ON PLANT AND SLIPPED AND FELL ON ROCK BERM THAT EQUIPMENT PARKS AGAINST AND SPRAINED HIS LEFT FOOT.

EE WAS ATTEMPTING TO DISMOUNT A EUCLID, LOST HIS FOOTING AND FELL TO THE GROUND.

EMPLOYEE SLIPPED ON BACK OF HAUL TRUCK WHILE CLEANING BACK WINDOW AND STRUCK HIS RIGHT KNEE ON A GREASE FITTING. LACERATION.

EMPLOYEE WAS CLEANING CONVEYER BELT, WHILE GETTING DOWN HE JUMPED A DISTANCE OF 4 FT TWISTING HIS RIGHT ANKLE.

EE WAS WALKING TOWARDS KOMATSU LOADER. HE SLIPPED ON ICE ON THE GROUND AND FELL FLAT ON HIS BACK.

EMPLOYEE WAS GETTING OUT OF WATER TRUCK AND MISSED LOWER STEP. EMPLOYEE FELL, TRIED TO CATCH HIMSELF AND SPRAINED RIGHT WRIST.

EE WAS GETTING A TOOL FROM THE BACK OF A TOOL TRUCK. THE EE JUMPED OUT OF THE BACK OF THE TOOL TRUCK SPRAINING HIS RIGHT ANKLE.
