While climbing up the rock transfer tower stairs employee felt a sharp pain in his right knee. Employee later went to the Dr. and was informed that he tore tendons.
EMPLOYEE WAS NOT USING HAND DOLLY FOR MOVING CYLINDERS. EMPLOYEE WAS CARRYING CYLINDER TO TOP OF 9-PACK. HE TRIPPED ON EDGE OF LAST STEP CAUSING EMPLOYEE TO FALL ON LEFT KNEE - SUSTAINED CUT TO LEFT KNEE.
Unattentiveness, not watching where walking, no keeping three-points of contact while descending stairs. While descending stairs employee was carrying items, looked away, missed the bottom step and fell forward onto ground. Abrasion to right knee and contusion to left hand. (Not keeping three points of contact and taking eyes off direction of travel.)
Late Report of Injury to Employer: Injury occurred on 4/2/09 but was not reported to management until 4/7/09. Employee states he was walking around the Coal Mill when his left ankle rolled over due to walking on uneven ground. Sprain to left ankle.
LATE REPORT OF INJURY BY EMPLOYEE- REPORTED ON 2/16/09. EMPLOYEE STATED THAT AS HE WAS PULLING ON THE BASE OF A STOP SIGN, WHICH WAS BENT, THE BASE BROKE CAUSING HIM TO FALL TO THE GROUND LANDING ON HIS LEFT ELBOW. (STRAIGHTENING OF THE STOP SIGN POST WAS NOT A JOB ASSIGNMENT BY THE SUPERVISOR. THE EMPLOYEE ELECTED TO PERFORM THIS TASK ON HIS OWN.)
While running up the stairs with both hands full employee mis-stepped causing him to stumble and banging his right wrist on the hand rail and twisting his back. (Lay off planned for 12/12/08)
While employee was exiting the cab of the water truck he missed the hand grab on the cab, not keeping 3-points of contact, and fell to the ground. Employee sustained abrasion and hairline fracture to right elbow. Employee was placed on modifed duty on 9/29/08 until he was seen by ortho dr. and then placed off duty.
Employee was cleaning #13 rock belt when he slipped and sprained his R/ ankle.
Employee was servicing a filter while standing on an 8 ft. ladder. The ladder slipped and he fell approximately 5 ft. landing on his right shoulder and dislocating it.

On 11/24/06 employee was working on a crane boom. A crane hook was coming down in the vicinity and the employee perceived the hook might strike him. The employee jumped down, landing on his feet and rolled forward.
Employee reports while stepping down of off 50 ton RT crane using the crane steps & hand rail, his right foot was caught under the step's rung, causing him to lose balance. He held onto the handrail, however the shift in balance caused him to overexert the right knee.
On 10/20/06 Employee was positioned below another employee, employee overhead fell and landed on employee below. Employee below reported back pain.

WORKING ON #3 COOLER, WHEN HE SLIPPED AND FELL. THIS WAS REPORTED AS A FIRST AID BACK STRAIN. ON 11/19/03 REQUESTED MEDICAL ATTENTION AND WAS PLACED ON MODIFIED DUTY.
THIS EMPLOYEE CLAIMS THAT AT 1:00 PM, 1 1/2 HRS BEFORE HIS SHIFT STARTED HE SPRAINED HIS RIGHT ANKLE WHEN HE STEPPED DOWN WHILE LEAVING TIME CLOCK HOUSE.
CLIMBING UP AND DOWN LADDER, LEFT KNEE SWELLED UP.
EMPLOYEE WAS WALKING BY #22 CLINKER BELT WHEN HE STEPPED ON LOOSE CLINKER. HE SLIPPED AND FELL, SPRAINING HIS BACK.
EE STATED THAT HE WAS GETTING OFF OF D-7 DOZER WHEN HE SLIPPED AND CAUGHT HIMSELF WITH LEFT HAND AND IN DOING SO. HE STRAINED HIS LEFT SHOULDER. THIS WAS REPORTED AS A FIRST AID INJURY AND A R EQUEST WAS MADE ON 6-23-03 TO HAVE A DR EXAMINE IT. HE WAS RETURNED TO WORK ON MODIFIED DUTY.
TRUCK DRIVER CUSTOMER WAS TYPING HIS LOAD. HE WAS STANDING ON HIS TRAILER, PULLING ON ROPE TO TIGHTEN, WHEN THE ROPE BROKE, CAUSING DRIVER TO FALL.
EMPLOYEE WAS STANDING ON A 4 FT LADDER, WHEN AN ELECTRICAL EXTENSION CORD BECAME TANGLED WITH HIS LEFT FOOT CAUSING HIM TO FALL.
