Operator of an excavator climbed up onto the machine to enter the cab. While standing on the track to enter the cab the operator fell backwards to the ground. operator dislocated their left shoulder, fractured their left hip and cut their left elbow requiring stitches.

EE was climbing down from 773D haul truck and EE's hand slipped off the handrail and fell approximately 10' to the ground injuring right wrist and back.
Employee was moving a heavy duty jack stand when EE lost balance and fell on the concrete floor of the maintenance shop. The jack stand landed on EE's right hand causing a laceration that required 9 stitches.
EE was not feeling well & sat down on a picnic table within the quarry shop. Shortly after sitting down, EE experienced a medical episode, believed to have been related to diabetes, which caused EE to faint & hit head on the picnic table. Incident was not work related but due to a medical episode. EE received staples to back of head that was cut when EE fainted & hit head on table.
Employee was changing screen cloth on the #1 screen when EE lost EE's balance and fell hurting EE's right knee and left shoulder. The employee continued working without restrictions until lay off when EE had surgery to repair EE's knee. Was originally first aid.
Injured employee slipped on the ice fracturing left leg.
Employee slipped on icy surface hurting right shoulder, continued to feel pain and was found to have shoulder problems requiring surgery on 04/11/2019, was originally reported as 1st Aid. Is now lost time.

Employee was climbing down the ladder of a quarry haul truck, when his foot slipped and he hit his left leg on a metal step of the haul truck, producing a laceration of the left leg just below the knee. Employee required 4 sutures to close wound.
