EE pulled up to quarry office to sign in for the day. As EE stepped out of truck onto the shoulder of road EE slipped on ice exiting the truck. EE tried to grab door but fell to ground with shoulder/right arm pain and was taken to urgent care.

EE (TRUCK DRIVER) CLIMBING INTO TRUCK WHEN DOOR HANDLE DISLODGED FROM DOOR AND LEFT KNEE STRUCK RUNG OF LADDER. EE DID NOT INFORM US HE NEEDED MEDICAL ATTENTION WE FOUND OUT AFTER HE WENT TO MEDICAL HELP ON ABOUT 8-26-03 WHEN BILLS ARRIVEDAT OFFICE.
