Showing all 3Miner turned/sprained his ankle while installing a screen. Miner did not report incident until Monday May 10, 2010. Miner stated he felt he was OK on May 6 and May 7 but his foot begin to swell up on Saturday May 8 so he sought medical treatment over the weekend and received a gel splint to help immobilize the foot. Miner was released back to full duty the same day.
12-6-07 employee slipped on ice straining his left knee. Employee did not seek medical treatment. 12-11-07 employee reported that knee was not improving, supervisor took him to local clinic. Employee was examined, diagnosed with knee strain, released same day, told he could continue to operate water truck, but should undergo 4-8 sessions of physical therapy.
