EE stepped on a slick spot resulting in a jerking motion in EE's back. At the time of reporting the incident, the discomfort was not bad, but increased over the next few days. When EE came into work on 9/7/21, EE requested to see a doctor that the pain had gotten worse and wanted checked out. EE was seen at the doctor on 9/8/21 and placed on restricted duty.

Showing all 20Employee was climbing down a ladder from the head box of a conveyor, when EE stepped off the ladder EE twisted EE's knee. EE did not seek medical attention until EE reported to work on 11/19/20. EE was taken to a doctor and was wrote out of work with a brace on EE's knee and no walking or climbing until follow-up on 11/24/20
Employee clocked and drove to kiln employee parking area and the proceeded to walk to the Kiln control room. As he stepped onto the first catwalk located under the Unit train conveyor # 2 just outside Unit train stone tunnel his right foot landed wrong on the walkway turning his right ankle.
Employee was coming down off the 334 coal belt. When he stepped off the pad onto the ground he stepped on ice and fell causing him to suffer a broken leg.
Employee was cleaning his work area at the primary crusher when he slipped and struck his leg. This caused an abrasion/hematoma. Initial check found no breaks. He returned to work with NO restrictions. On the 27th he reported to the hospital and was found to have an infection in the leg. He was given restricted duty at this time.
Fell as he was coming down a sloping slippery catwalk. Grabbed for handrail and caught edge of metal building, cut 4th finger on left hand.
EE was trying to access the front of the scaler when he slipped on some spilled oil on the hydraulic tank and fell approximately 4 feet to the ground, landing on his left side. EE did not lose any time until 10/7/05 when he had a partial lateral menisectomy performed on his left knee. --EE complained that knee still was not feeling good and started losing time again.5/19/06
Employee was hunched down near the ground trying to get a return idler to move by hitting it with a hammer. When it started moving he fell forward and put his hands in a pile of hot lime. He was wearing gloves and long sleeves but the lime got in between the two and caused second degree burns on his right wrist.
Employee was replacing 2 of 5 pieces of 3"conduit that were in an 18" ditch.The ditch was extremely wet.The conduit had 3-4" gaps between each piece. While stepping on the conduit to climb out of the ditch the employee's foot slipped between conduit twisting right knee.Employee tore his miniscus and will be having surgery on 11/18/04 which will be his first day off.
EE WAS IN THE PROCESS OF MOVING RAIL CARS UTILIZING A RAIL CAR PULLER. THE PULLER DID NOT STOP THE RAIL CARS FULLY & EE TIGHTENED BRAKE ON THE BACK OF THE 2ND RAIL CAR. EE WENT TO WALK OVER TH E RAIL CAR PULLER CABLE WHEN HE SLIPPED IN SOME MUD. AS HE WAS FALLING THE CABLE TIGHTENED, COMING UP OFF THE GROUND & STRUCK EE IN THE FACE. THE EE NEEDED 4 STITCHES IN HIS LIP.
INJURED IS A CUSTOMER TRUCK DRIVER ON SITE TO GET A LOAD FROM THE HYDRATE LOADOUT. HE WALKED ON A CONCRETE SIDEWALK FROM THE CAB OF HIS TRUCK TO THE BUCKET THAT HE PUT HIS LOADOUT TICKET IN. H E SLIPPED & FELL ONTO HIS LEFT LEG & HIP. THE SIDEWALK HAD APPROX. 1/4" WET HYDRATE ON IT. HE PREVIOUSLY HAD HIP SURGERY. PRELIMINARY DOCTOR REPORTS ARE CALLING THE INJURY A SEVERE BRUISED HI
EE WAS WALKING INSIDE KILN WHEN HE STEPPED IN A HOLE CREATED BY WORN BRICK AND GATORS. EE LOST BALANCE AND TWISTED HIS BACK IN THE PROCESS TO REGAIN BALANCE. EE WAS WEARING DARK SAFETY GLASS ES IN A DARK LOCATION AT THE TIME OF INCIDENT.
EE WAS PUTTING ON A HARNESS WHEN HIS FOOT SLIPPED OFF A ROCK TWISTING HIS LEFT KNEE. AN MRI WAS SCHEDULED & HE WAS RETURNED WITH NO RESTRICTIONS BY THE DOCTOR TO COME BACK TO WORK. AFTER THE M RI WAS DONE IT WAS FOUND THAT THE EE HAD A TORN ACL & WILL HAVE SURGERY.
EE WAS WORKING TO REMOVE OLD STOPS IN PRIMARY JAW CRUSHER. JAW CRUSHER WAS OUT BEING REPAIRED.EE NEEDED TO GET OVER A BEAM IN ORDER TO STEP ON A SMALL PLATFORM TO DO HIS WORK. HE STEPPED ON THE BEAM WITH HIS LEFT FOOT. UPON RAISING HIS RT. FOOT OFF THE GROUND HE SLIPPED AND WENT HEAD FIRST IN THE OPENING CREATED BY THE MISSING JAW CRUSHER. EE FELL 13' TO THE CONCRETE BELOW.
EE COMPLAINED THAT HIS SHOULDER WAS HURTING FROM HIS ACCIDENT ON 10-4-01. HE WENT TO SEE DR AND IS ON RESTRICTED DUTY.
THE DRIVER GOT OUT OF HIS TRUCK AND WAS WALKING TO HOOK UP DUST COLLECTOR HOSE. HE SLIPPED WHILE CROSSING OVER SOME PALLETS. WHEN HE FELL, HE HIT HIS HEAD ON A LARGE ROCK. HE WAS TRANSPORTED T O HOSPITAL.
EE WAS POURING ANTIFREEZE INTO TRACKHOE WHEN HE LOST HIS BALANCE AND FELL TO THE GROUND. HE WAS UNCONSCIOUS FOR 3 TO 4 MINUTES.
EE WAS WALKING UP THE STEPS INSIDE THE OFFICE WHEN HE TWISTED HIS FOOT CAUSING A FRACTURE TO A BONE IN HIS RIGHT FOOT.
INDIVIDUAL WAS STANDING ON A CONVEYER BELT SHOVELING THE STONE OFF. HE LOST HIS BALNCE OR SLIPPED AND FELL APPROX 10 FT, HITTING HIS HEAD ON A HAND RAIL BEFORE LANDING ON THE GROUND. HE SUSTAI NED A LACERATION ON HIS RIGHT FOREHEAD OF APPROXONE INCH AND A DISLOCATED RIGHT SHOULDER. THERE WERE MINOR ABRASIONS TO BOTH SHINS.
INDIVIDUAL AND FELLOW WORKER WERE MOVING AN ALUMINUM PORTABLE CONVEYER BACK FURTHER IN THE KILN.THE INJURED STUMBLED AND FELL AND MASHED HIS HAND AGAINST THE OLD BRICKS AND THE CONVEYER.
