EE ON 10/15/97 REPORTED HE HAD SLIPPED AT THE BASE OF THE 1'A CONVEYOR & FELL INTO THE SKIRTBOARD. [NO LOSS OF TIME OR MEDICAL REPORTED]. WHILE ON WINTER LAYOFF EE WENT TO DOCTORS [APPROX 4 TI MES] WITHOUT OUR KNOWLEDGE. WE WERE AWARE OF THESE VISITS ON 4/16/98. THERE HAS BEEN NO LOSS OF WORK.
