EE was looking at #7 scaler boom connection & when walking forward, stepped on a slick spot & fell backwards on his back, using his arms to block the fall. EE did not express a need for treatment at the time of the incident. After experiencing shoulder pain, EE sought medical attention & was diagnosed with a torn tendon, requiring surgery.

Showing all 29Employee reported injury on 5/11/10 that he had sprained his ankle getting out of operator compartment of lube truck. He continued working his regular shift. He There were no major complaints up through mid-July. He officially began missing work on 7/23/10 when he was scheduled for surgery.
Employee was working in oil service bay filling lube truck and stepped on slick bottom, fell, and twisted knee.
Employee was walking in the UG shop wash bay when he stepped into an eroded area of the salt mine floor. His ankle creeled and he fell. He was in the process of washing a piece of equipment with a pressure washer.
Employee was on a ladder disconnecting the cylinder to install cables on a 1200 JLG man lift. He fell from the ladder and suffered a laceration to the elbow and a contusion of the sacrum.
Employee creeled ankle and suffered severe sprain when stepping down from a 9-12" offset on workplatform.
Employee was assigned to clean up oil spillage near the block press. While applying oil to the dry floor, he slipped and grabbed a stack of pallets with his left hand to avoid falling to the ground level. Later in the shift he observed a knot that formed on his left wrist as a result of grabbing the pallets.
Employee slipped on slick floor and caught himself before falling to the ground. While slipping he twisted his knee. Employee continued to experience problems and because of his chronic arthritis condition required surgery (ACL repair) to the knee. THIS IS NOT A NEW INCIDENT.
Employee states he was walking around the back of a fork truck at the maintenance shop to access the area he was working in. While walking behind the fork truck and through the bay door, his right foot turned on an elevated part of the asphalt walking surface causing him to fall to ground level.
Employee suffered injury when part of handrail broke loose on side of elevated platform in Crushing/Screening plant. As he put hand on railing, it separated, causing him to fall onto the platform. He put his arm out to help lessen fall and injured arm.
EE stated she slipped going down wooden steps & fell, landing on her rt arm & buttocks. She alleged pain in her back & neck area. After being examined at the local hospital ER, she was released to return to work. In interview that day & afterward, she stated she was ok. She began missing work on 4/3/09 for unknown reason at the time. Mangagement feels this is not reportable.
Employee was descending an extension ladder when the ladder shifted on it's base and caused the employee to lose his footing. He fell off the ladder from about the fourth rung from the bottom of the ladder. In the process he suffered a fractured right wrist when he contacted the ground.
EE states he was bagging 40lb film sacks of pellets, when he stepped off the wooden platform to retrieve more bags. While stepping down from the platform, his foot contacted the edge of the wooden platform, causing him to lose his balance and fall forward. He raised his right arm and grabbed the ink jet coder box to keep from falling. Strained right ankle and arm below shoulder.
Employee injured on 3/27/08 when getting out of skid-steer loader. He slipped and leg went between bucket and loader frame. Took Family Medical Leave for some reason on 3/28/08. Worked on 3/31 & 4/1. Went to doctor on 4/2/08 and given restricted duty.
Employee has just finished closing a barge cover. He turned to exit off the top of the barge, slipped and slid off the side of the barge between the bulkhead and the barge.
EE was attempting to remove a defective product (can of salt - approx. 1 lb.) from a conveyor belt when she lost her footing, slipped and fell to the floor at ground level.
Employee was injured when he lost footing on wet slippery area of mill floor. When he slipped and began falling, he extended his arm to help "break" his fall. THIS INJURY REPORTED AS RESTRICTED DUTY.
EMPLOYEE WAS INJURED WHEN HE SLIPPED AND FELL WHILE PICKING UP A UTILITY TOOLBOX IN AN AREA OF UNEVEN/SLICK MINE FLOOR. AS HE PICKED UP THE TOOLBOX, HE LOST HIS FOOTING, DROPPED THE BOX AND STRUCK IT AS HE FELL.
EE was disembarking the volvo 150 loader and slipped from the bottom ladder rung. he managed to break his fall with his hands, spraining his left wrist. ee experienced swelling in his wrist and further medical evaluation on 5/5/04 revealed an avulsion.
ON 9/16/03, EMPLOYEE HAD CLIMBED DOWN A LADDER AND STEPPED TO THE GROUND. AS HE TURNED TO WALK, HE TRIPPED AND FELL. THIS INJURED HIS RIGHT SHOULDER. EMPLOYEE SOUGHT MEDICAL ATTENTION AND WAS RELEASED FOR NORMAL DUTIES. AFTER CONTINUING TO EXPPERIENCE PROBLEMS WITH HIS SHOULDER FURTHUR MEDICAL EVALUATION DETERMINED THAT SURGERY WOULD BE REQUIRED. OUTPATIENT SURGERY - 1/9/04.
DURING BUCKET ELEVATOR REPLACEMENT, EE WAS PUSHING A CRANE CABLE USED TO SUPPORT THE WEIGHT OF THE ELEVATOR CHAIN. THE CABLE SUPPORTING THE CHAIN WAS NOT FULLY TENSIONS. AS CHAIN CAME OFF THE SPROCKET, THE CHAIN MOVED DOWNWARD CAUSING EE TOSLIP, LOSE BALANCE AND FALL FORWARD AND CONTACT THE METAL ELEVATOR HOUSING.
EE WAS INJURED WHEN HE SLIPPED ON LOOSE MATERIALAS HE WAS WALKING AROUND THE CUTTING. *EE FURTHER AGGRAVATED THIS INJURY ON 10/7/99AND5/20/02.
EMPLOYEE RECEIVED AN INJURY WHEN HE FELL GETTINGOUT OF THE UTILITY RIG BASKET. WITH THE RIG SITTING ON AN INCLINE DUE TO FLOOR HEAVE, THE BASKET WAS MOVED TO THE SIDE OF THE MACHINE TO EXIT THE BASKET. HE RAISED THE BASKET GATE BAR TO EXIT HOLDING ONTO IT FOR SUPPORT. THE HINGE END OF THE BAR BROKE UNEXPECTEDLY AND HE FELL.
EE WAS INJURED WHILE WALKING FROM CHANGEROOM TO SHOP WORKSITE AT START OF SHIFT. WALKING THROUGHA WELL-LIGHTED AREA, HE STEPPED ON UNEVEN ASPHALT AND TURNED HIS ANKLE, NOT FULLY FALLING TO THE GROUND. INJURY REPORTED AS RESTRICTED DUTY.
EE REP'D STEPPING ON SOMETHING & FALLING WHILE WALKING IN THE PARKING LOT. FOLLOW UP MEDICAL ATTENTION NOT AVAILABLE UNTIL 11/14/01. INCIDENTBEING REPORTED AS RESTRICTED DUTY
WHILE CHANGING A TAKE-UP CABLE ON THE 11 BELT TAKE-UP, THE EMPLOYEE SLIPPED AND FELL ON HIS KNEES. EMPLOYEE VISITED MED WORKS AND WAS RELEASED TO FULL DUTY ON 5-7-01. EMPLOYEE CONTINUED TO HAV E DISCOMFORT, RETURNED TO MED WORKS AND AN MRI WAS SCHEDULED. THE MRI REVEALED A TORN CARTILAGE WHICH WILL REQUIRE ORTHOSCOPIC CORRECTION (5-25-01) AND LIGHT DUTY(5-29-01).
EMPLOYEE WAS RUSHING TO THE WAREHOUSE TO AVOID BEING LATE FOR WORK. AS HE WAS CROSSING THE RAILROAD TRACK THE HEEL OF HIS SHOE SLIPPED OFF THE TOP EDGE OF THE RAIL DOWNWARD CAUSING THE INJURY
EE WAS WALKING DOWN A SLIGHT GRADE AND LOST FOOTING ON THE SLICK MINE FLOOR WHICH CAUSED HER TO FALL. THIS INJURY IS BEING REPORTED AS RESTRICTED DUTY CASE.
EMPLOYEE WAS ASSISTING IN PULLING A BULKHEAD DOOR IN A RAILCAR IN PREPARATION FOR LOADING. WHILEPULLING ON THE DOOR, HER RIGHT FOOT SLIPPED AND BECAME CAUGHT BETWEEN THE BOTTOM OF THE BULKHEAD DOOR AND RAILCAR FLOOR.
