Miner was dismounting ladder on 980 G FEL. Right knee buckled when miner placed right foot on ground. Miner was on flat clean surface.
Miner was stepping out of bed of service truck onto bumper, slipped and hit the ground on right shoulder. Miner was taken to doctor to be checked out and was issued prescription for anti-inflammatory.
Miner was exiting plant control tower coming down steps, and slipped on wet step. Miner reported incident on day of occurence, but refused medical attention at time of incident. WHILE CHECKING ON PUMPS AROUND POND, STEPPED ON SOME SHOT ROCK AND CAUSED KNEE TO TWIST. HE COULD FEEL HIS KNEE CAP POP. WENT HOME AND HAD HIS WIFE TAKE HIM TO THE HOSPITAL.
WALKING ON CATWALK, TRIPPED ON VALVE HANDLE, FALLING AND CATCHING LEFT FOREARM ON TOP ADGE OF SCREEN THAT WAS LEANING UP AGAINST THE HANDRAIL OFTHE WET SCREEN, LACERATION OF LEFT FOREARM.
