During roof prep work, contract employee stepped through a roof hole while moving a piece of plywood that was covering the hole. He was secured with fall protection but suffered leg strain when he leaned back to restore his balance. He was placed on modified duty for three days.

Showing all 13Employee was exiting control booth and slipped on the dry, 3 step stairway while descending. He caught himself before falling and complained of pain in his knee. Was seen and determined to have a slight strain. Employee was sent back to work with no restrictions. This became a reportable incident on 1/3/13 when treatment went from first aid to medical treatment by definition.
EE claims he slipped on wet material while attempting to fill a 5 gallon pail with lube in order to start his lube duties. EE was seen limping into work and this accident is under investigation for validity.
EE was walking up out of a shallow bin area when his foot slipped and he grabbed onto the wall to keep from slipping further. He sprained his shoulder during this action, resulting in restricted duty work.
There was lots of snow and ice on the ground.
Ee stepped in what he thought was a solid surface, which was, in fact, not solid. Overcompensation for balance lost caused ee to jerk backwards thereby spraining back muscles and spraining/straining Left knee and leg.
EMPLOYEE WAS WALKING IN AISLE WHEN HE TRIPPED OVER A BROKEN, PROTRUDING PIECE OF WOOD STICKING OUT FROM A PALLET. HE TRIPPED AND FELL HURTING HIS RIGHT SHOULDER, UPPER ARM, CUTTING HIS MIDDLE FINGER ON RIGHT HAND, AND CONTUSIONS OF LEFT KNEE AND LOWER LEG.
GROUND WAS WET FROM RECENT RAIN & EE WAS IN A HURRY
WE HAD BEEN HAVING BAD WEATHER, CONDITIONS WERE SNOW AND ICY, EMPLOYEE WAS WALKING OUT OF BUILDING AND DOWN RAMP TO FORKLIFT. SNOW FALLING ON FROZEN SURFACE MADE THE RAMP SLIPPERY.
WE HAD A SEVERE ICE STORM AND WERE CAUGHT WITHOUT PREPARATION.
WHILE WALKING THRU AREA USED TO STRAP SEMI BULK BAGS TO THEIR PALLETS, EE S FOOT GOT CAUGHT ON STRAPPING MATERIAL INT HE AISLE WAY. EE FELL TO HIS LEFT KNEE, CATCHING HIMSELF WITH HIS RIGHT HA ND TO PREVENT TOTAL COLLAPSE.
WHILE DISMOUNTING FROM BULK TANK TRUCK, FOOT SLIPPED AND ANKLE TWISTED.
EE REMOVED SECTION OF CATWALK TO WORK ON SECTIONOF PIPE WHILE MOVING AROUND EE INADVERTENTLY FELL INTO OPENING
