WHILE CLIMBING LADDER (POWER SCREEN) EE STRUCK HEAD ON LOW CLEARANCE. THIS CAUSED HIM TO LOSE BALANCE AND FALL TO THE GROUND WHERE HE STRUCK HIS HEAD ON A PIECE OF STEEL. HE RECEIVED 2 LACERATIONS ON HIS HEAD WHICH REQUIRED MEDICAL TREATMENT.

EE WASS CONDUCTING PRESHIFT INSPECTION ON HAUL TRUCK. TRIPPED ON EDGE OF 3/6" STORE PILE. EE SUFFERED CONTUSION ON RIGHT SHOULDER.
