Employee was entering the wash tunnel. Employee lost their footing on staircase and fell forward impacting their hand on the concrete floor.

Employee was walking to the shop door which was locked so employee walked around to the south side of the shop and slipped and fell on ice hitting head and straining shoulder.

Employee was standing on a ground level catwalk, swapping out frozen water lines to the dust suppression system, when EE stepped backward rolling EE's ankle causing a fracture and dislocation to the ankle.

A miner was exiting a MCC connex which consisted of 1 step down to ground level. The Miner mis-stepped on the level ground and rolled EE's ankle.

Employee was walking across the 8x20 top deck screen and stepped on a soft spot on the screen media with the outisde of his right foot, while the inside of his foot was on a ridgid support beam, causing his foot to roll to the outside and causing an ankle sprain.

Employee was climbing a ladder. As he got to the 4th rung, the ladder slipped out from underneath him. He tried to grab a picking eye to regain balance but could not do so and slid to the ground.

Employee was kneeling on CBI, throwing rocks off. He lost balance and fell landing feet first.

EE was stepping out of the cat 226B skidder with the bucket full of material and with the lip of the bucket tip up in the air. As he stepped out he tripped on the bucket and fell to the ground.

He finish removed snow and parked. Got into a 3 pt stance & had both feet on the top step, moved his right foot front the 1st step to 2nd step his L foot slipped off the top step. tried to catch himself with his hands lost his grip & fell to ground on his right side fracture his r pelvis. 24% ft & quit snowing at time of accident. He had cleaned light snow off top deck with feet,

Ee was walking out of cb2 tunnel when he slipped on ice.

As employee was removing guards, he planted his foot on some mud/fines. His foot sank, quickly losing balance, shifting his momentum forward, resulting in employee striking the top of his hard hat against the plant structure.

Employee was exiting loader when he slipped and fell hurting his left leg.

While installing a roller, employee slipped and fell hitting his lip on a steel cross over.

EMPLOYEE WAS GETTING OUT OF THE WATER TRUCK WHEN THE DOOR STOP STRAP BROKE CAUSING HIM TO STEP DOWN BEFORE HE WAS READY. HE TWISTED HIS LEFT ANKLE AS A RESULT

EMPLOYEE SLIPPED WHILE CLIMBING DOWN STAIRS. FELL 1 STEP TO CATWALK AND BRUISED CHEST. RESTRICTED DUTY. * EMPLOYEE REPORTED INJURY ON 4/27/05 AND REQUESTED TO SEE DR. 4/28/05*

Employee turned to speak with supervisor, slipped on ice and fell down. Note: employee worked 11/30 to 12/01, saw physician on 12/02; work restricted 12/02 to 12/04/04.

WHILE CLIMBING LADDER (POWER SCREEN) EE STRUCK HEAD ON LOW CLEARANCE. THIS CAUSED HIM TO LOSE BALANCE AND FALL TO THE GROUND WHERE HE STRUCK HIS HEAD ON A PIECE OF STEEL. HE RECEIVED 2 LACERATIONS ON HIS HEAD WHICH REQUIRED MEDICAL TREATMENT.

EE WAS WALKING ON A LEVEL HARD SURFACE WHEN HE LOST HIS FOOTING AND FELL DOWN. HE TRIED TO CATCH HIS FALL WITH HIS LEFT HAND AND ENDED UP WITH A FRACTURED LEFT WRIST. INVESTIGATION SHOWS NO TRIP HAZARDS PRESENT.

EE WAS STEPPING DOWN OFF OF A CONVEYOR FRAME. HE PUT HIS FOOT DOWN WRONG & TWISTED HIS RIGHT ANKLE.

EE WAS WALKING ON UNLEVEL DIRT DRIVE WHEN HE SPRAINED HIS ANKLE.

EE WAS DOING MAINTENANCE WORK ON HP 400 CRUSHER. EE WAS WORKING ON DECK AREA AND TRIPPED ON A ROCK THAT WAS ON DECK, FELL BACKWARD INJURING AREA BELOW RIGHT SHOULDER.

EE WAS CRAWLING UP ON A CONVEYOR BELT WHEN HIS PANT LEG GOT CAUGHT ON THE FRAME. HE FELL BACK ON THE HANDRAIL LANDING ON HIS LEFT SIDE.

EE WAS WALKING FROM THE SHOP TO CAR WHEN HE SLIPPED ON THE ICE AND SNOW.

EMPLOYEE WAS TRYING TO HAMMER IN A ARM PIN ON HIS LOADER HE HAD ONE FOOT BRACED ON THE FINDER AND ONE ON THE FRAME WITH HIS BACK BRACED AGAINST THE HYD TANK AND SLIPPED AND FELL.

EE WAS WORKING ON A CATWALK THAT HAD A TRAP DOOR TO ANOTHER LEVEL. INSTEAD OF CLOSING THE DOOR THE EE HAD BEEN STEPPING OVER IT WHERE THE ACCIDENT OCCURRED HE STEPPED BACK AND FELL PARTWAY THR OUGH.

EE WASS CONDUCTING PRESHIFT INSPECTION ON HAUL TRUCK. TRIPPED ON EDGE OF 3/6" STORE PILE. EE SUFFERED CONTUSION ON RIGHT SHOULDER.
