Showing all 13Employee was attempting to push a rock that was on the haul road. While doing so, the employee slipped and hit EE's chin on the rock.
EE stepped through a rotted piece of flooring in front of the furnace in the Ogden scale house. It was found that the furnace condensation line had been leaking directly under the furnace and rotted the plywood flooring adjacent to the furnace.
EE walking across parking lot and states right foot slid out from underneath EE and EE braced with EE's left foot to stop from falling. Bracing caused pain to left ankle, was sent to Urgent Care the following day. EE was put on light duty until EE could receive a follow up two weeks later. There was a break in the ankle and set to remain on light duty until end of September
