EE slipped on a patch of ice falling onto EE's right hand that was holding a hammer. EE fractured the middle, ring, and pinky fingers of right hand.

Showing all 23While descending from a roof bolter, EE planted EE's right foot at the edge of the cab when EE lost EE's footing and missed the top step, causing EE to fall approx. 5 ft. onto EE's right shoulder. On 6/3/25 EE was evaluated at Ohio Health Work Health where EE was diagnosed with a strain and given pushing/pulling/lifting restrictions of 10lbs with right arm.
Truck driver was walking on a blasting bench. It had rained so the bench was muddy and had some standing water. The driver stepped in a hole that was obscured by water and twisted left foot/ankle.
A miner fell from a height of approximately 9' from a fixed conveyor catwalk while clearing a tail guard. The miner had no fall protection while using a pry bar measuring 70" and 17 lbs while standing on a fiberglass ladder and the fixed mid-rail. The miner was taken to the ER and received 8 staples to a laceration on EE's head, was given a sling for a strain to EE's shoulder.
Employee was working on a crusher and climbed onto the crusher's drive belt guard without the required fall protection. Employee slipped and fell 6 feet onto the crusher platform. The fall resulted in a fracture to the pelvic bone, the employee was released with restrictions.
Employee was walking the plant. While descending the stairs from the 4 1/4 crusher to the floor below EE slipped and fell onto the steps. Employee struck left elbow on the edge of a step causing a laceration to the elbow.
Employee was operating a Gradall 4310 Mine Scaler in an underground room and pillar mine. Employee stopped the machine and shut it off. As the employee exited the machine he missed a step with his right foot causing him to slip and fall approximately 4 1/2 feet to the ground. When he hit the ground he twisted his left knee resulting in a soft tissue injury diagnosed as a sprain.
Approx. 10:30am local authorities discovered a man committed suicide by jumping from a public bridge which landed approx. 90ft below in our open pit mining site. MSHA was notified immediately upon discovering the body and an inspector was on our site within a couple hours to conduct a thorough investigation which resulted in no changeability. The police report # is 13-0373.
EE attempted to climb into the bed of his sterling dump truck when he lost his balance and fell to the ground. EE suffered a broken left arm (upper). EE was transported to Mt. Carmel west hospital.
EE moved from one side of conveyor tail pulley to the other. When he put his foot down, his knee gave out on him. He didn't bump it on anything.
Employee was preparing to cleaning spilled material under 304 conveyor tail pulley with water hose. When he turn on the water hose, water presure caused him to lose his balance and fall backwards against the steps leading to the screen tower and then to the ground landing on his right hand.
After employee finished his preshift plant inspection, employee went to the port-a-john. After exiting, employee walked 4-5 feet and tripped over a rock, and landed on his right knee, causing the dislocation.
EE STEPPED OUT OF HIS TRUCK ON THE WALKWAY AT THE SCALES. HE WEARS BI-FOCAL GLASSES AND MISSED THE FIRST STEP FALLING 30' TO THE CONCRETE SIDEWALK. HE HIT HIS RIGHT ELBOW ON THE SCALE BEAM CUTTING HIS ELBOW (NO STITCHES REQUIRED) AND BRUISED HIS LEFT HIP WHEN HE HIT THE CONCRETE, HE ALSO PULLED HIS LEFT BICEP.
Chute from 8'x20' screen to surge rock transfer conveyor plugged up w/material & had to be cleared. Once plug up was cleared EE & co-worker were walking under screen tower to haul trucks. A large wood block was on the concrete. EE saw the block & tried to step over it. He tripped & began to fall. He reached out & grabbed the side of the conveyor to keep from falling.
EMPLOYEE WAS GREASING A LOADER WITH AN AIR GREASER LOCATED AT THE EQUIPMENT SHOP. THERE WERE SOME SHEEL CHOCKS AND A FEW STONES ON THE GROUND BETWEEN THE LOADER AND THE SHOP WALL. THE EMPLOYEE WAS WALKING BETWEEN THE LOADER AND THE SHOP WHEN HE TRIPPED OVER ONE OF THE STONES OR WHEEL CHOCKS AND FELL TO THE GROUND DISLOCATING HIS LEFT SHOULDER. HE WAS TAKEN TO THE HOSPITAL.
EE WAS STANDING ON BOULDER AND SLIPPED, FALLING AGAINST TRUCK AND INJURING HIS SHOULDER. (EE WAS LOADING BOULDER WITH EXPLOSIVES TO SHOOT IT. GRABBED DOOR HANDLE ON TRUCK AS HE FELL - HYPEREXTENDED HIS SHOULDER.)
2ND SHIFT WAS WORKING ON DREDGE PIPE AND AT QUITTING TIME THEY TOOK THE WORK BOAT TO THE BANK & EE STEPPED OUT OF THE WORK BOAT ONTO THE SIDE OF THE BOAT TIED TO THE BANK & HIS FOOT SLIPPED & HE FELL. HE HIT HIS SIDE ON THE BOAT AND CRACKED 6 RIBS.
HE STEPPED OFF THE BACK BUMPER OF THE WATER TRUCK. IT WAS NIGHT TIME BUT THE AREA WAS WELL LIT. WHEN HIS FOOT HIT THE GROUND IT WENT INTO A HOLE AND TWISTED HIS ANKLE CAUSING IT TO BREAK.
EE WAS WALKING TO STEEL RACK, STEPPED OVER 2' WALL, RT. LEG FIRST THEN HIS LEFT LEG, WHEN THE WEIGHT WAS PUT ON HIS LEFT LEG EE FELT PAIN IN HIS LEFT KNEE.
EE WAS CLEANING OUT CHUTE AT FEED HOPPER ON #3 DRY PLANT AND FELL 5'ONTO THE GROUND & LANDED ON A LARGE ROCK ON HIS LEFT SIDE. HE WORKED THE REST OF THE WEEK AND WAS O-K, SATURDAY HE WAS TAKEN TO THE HOSPITAL AND THEY FOUND HE HAD BROKEN HIS SPLEEN.
EMPLOYEE WAS STEPPING DOWN FROM SERVICE TRUCK & STEPPED INTO SNOW COVERED RUT CAUSING RIGHT ANKLE TO GIVE OUT.
EE WAS GREASING A ROCK TRUCK. HE CLIMBED INTO THE CAB TO GET A TUBE OF GREASE. WHEN HE BEGAN CLIMBING DOWN FROM THE CAB HE MISTAKENLY GRABBED THE WINDSHIELD WIPER ARM ISNTEAD OF THE GRAB HANDL E LOCATED BELOW THE WIPER ARM. THE WIPER ARM PULLED AWAY FROM THE TRUCK THROWING HIM OFF BALANCE, HE SLIPPED AND SLID DOWN THE TRUCK'S LADDER LANDING FORCEFULLY ON HIS FEET.
AFTER DOING REPAIRS'S TO KILMAN SCREEN, EE WAS CLIMBING DOWN FOOT SLIPPED OFF RAILING, LANDING ON THE SIDE OF LEFT ANKLE, (FULL WEIGHT).
