Parked the haul truck end of shift, climbed out proceeding down 3 point ladder. Stepped off ladder slipped on wet surface landing on her left elbow and twisted her right ankle. Time of fall she did not notice any pain. She reported to supervisor at 7:00PM of fall. She has been working everyday. Now has ankle swelling and a bruise elbow. She was sent to doctor 04/30/15 Thursday.

Showing all 3Employee stepped off of the ladders of a front-end loader and felt pain in right hip similar to a cramp. Pain did not susbside as a cramp would. Extreme pain after shift was reported to supervisor and medical attention was approved. No problems were observed with the ladder, seat or loader.
EE WAS CLEANING A TRUCK WINDSHIELD AND HE FELL HE TURNED HIS ANKLE WHEN HE LANDED. THE LACERATION WAS AT THE CONCRTE PLANT.
