EE WAS MAKING A ROUND CHECKING EQUIPMENT AND SLIPPED ON A PUDDLE OF GREASE THAT HAD BEEN LEFT IN THE MIDDLE OF THE FLOOR. HE TWISTED HIS RIGHT KNEE WHEN HE SLIPPED. HIS KNEE HAS GOTTEN PROGRES SIVELY WORSE. HE SOUGHT MEDICAL ATTENTION, AND HAD SURGERY TO CORRECT THE PROBLEM TORN CARTILAGE ON OCT 11, 2002 AT WHICH TIME THE ACCIDENT BECAME REPORTABLE.

EMPLOYEE WAS ON A PLATFORM UNPLUGGING A DUST LINE. HE FINISHED THE JOB AND CLIMBED DOWN THE LADDER. HE JUMPED FROM SEVERAL RUNGS UP AND LANDED ON A SLEDGE HAMMER THAT HAD BEEN LEFT ON THE FLO OR. HE BROKE HIS RIGHT LEG JUST ABOVE THE ANKLE.
