Employee alleges he strained his right groin area as he slipped while sweeping clinker and did a half spllit. He failed to report this accident until 12-15-09 at 8:50 AM. He was sent to the Industrial Clinic and was placed on restricted work duty. This alleged incident is being investigated as to whether or not it is work related or an incident that may have happened at home.

EE a Mine Engineer, was surveying the pit using a GPS system. He walked out 6' from the edge of the bench when the ground sloughed out from under him.

EE was walking up steps near K2 cooler when his foot slipped and he hit his face on the step, cutting his lip. The area was free of debris, water, or anything else that may have contributed.

Employee was stepping off the pinion stand to connect the ground when he twisted his left ankle. He was placed on restricted work.

Employee working on inverted cone. Employee slipped/tripped causing a minor injury to his right hand. Minor fracture to thumb/finger.

Employee was walking down stairs in K3 tower and pulled back muscle

Employee was preparing to move stacker from the west pile when he fell about 8 feet from the lower platform.

EE was assisting lowering of the boom on the American Crane when a sudden movement startled him causing him to step back. As he stepped back he tripped on a permanently mounted water jug falling to the ground below approx. 4 1/2 feet.

EE calims to have been exiting Loader 370 (992G) on back steps. EE claims to have fell from steps hitting left side of arm and back on steps.

Employee was dismounting from AHU395, lost his footing on the ladder and felt a pop in his right shoulder. Next day shoulder felt worse he and wanted to seek medical attention.

Internal audit found need to report. EE was walking from mill #11 to storeroom slipped on mill ball and injured knee. EE did not report injury for 7 days.

Internal audit found need to report. EE was dumping car #73 when EE was getting off the car #73 when he landed on left foot which rolled inward cause EE to twist his ankle.

EMPLOYEE WAS WORKING IN CLINKER STORAGE AREA. HE SLIPPED SEVERAL TIMES ON LOOSE CLINKER. THE REPEATED SLIPPING AGGREVATED A PREVIOUS BACK INJURY. HE DID NOT REPORT INJURY UNTIL 5/22/02.

EE BUMPED FOOT ON ABANDONED ELECTRICAL BOX CAUSING HIM TO LOSE HIS BALANCE AND HIT HIS LEG ON THE FORKS OF A FORKLIFT RESULTING IN CUTS AND ABRASIONS ON LOWER LEG LEG.

ENTRY INTO COOLER HAD WOOD PLANKS TO ALLOW WHEELBARROW MOVEMENT FOR REFRACTORY WORK. ENTRY WAS SLIPPERY FROM SAND & HE SLIPPED & FELL IN COOLER LANDING ON ROLL CRUSHER.

EMPLOYEE WAS INSTALLING A BEAM APP 20' OFF OF THE FLOOR BELOW AND FAILED TO TIE OFF HIS LANYARD AND FELL APP 20'.
