Showing all 11A Master Mechanic was servicing a D8 Dozer on the morning of 4/22/17. As the Master Mechanic stepped down from the tracks to the blade bar, right foot slipped on mud that accumulated on the bar, causing left foot to twist inward.
Employee was walking across a parking lot towards his haul truck when he slipped on a patch of ice and fell, resulting in sprains to his left knee and shoulder and abrasion to his left knee. Employee was placed on three days restricted duty, operator accommodated restrictions - no lost workdays were incurred as a result of this injury.
Employee removed lid from top of anfo truck and lowered hand rail on top of truck while on top. He then stepped bacwards and caught his foot in hand-rail. apparatus causing him to fall on belly auger of truck and truck rear bumper. approx. 8'
While palletizing flat rock miner slipped/ tripped over another rock and injured left ankle. Conditions were wet that day.
Employee was on scales. He got out of his truck and stepped on the guardrail they have for them to step on to get his load ticket. He turned to get back in his truck when his foot slipped on guardrail and he fell hitting his left side on guardrail and fracturing a rib.
WHILE EXITING THE LOADER THE EE MISSED THE BOTTOM STEP CAUSING HIM TO FALL TO THE GROUND. THE EESTATED HE HEARD A LOUD "POP" IN HIS KNEE. THE EE DID NOT SEEK MEDICAL TREATMENT UNTIL 5-28-03 AND THE ACCIDENT WAS REPORTED 5-29-03.
AS THE EE WAS EXITING THE HAUL TRUCK HE CAUGHT HIS FOOT ON THE DOOR PANEL AND TWISTED HIS KNEE.ON 4/1/03 HE WAS PRESCRIBED PHYSICAL THERAPY.
EMPLOYEE ORIGINALLY FELL ON ICE WALKING ACROSS A ROADWAY. HE WAS FIRST DIAGNOSED WITH A CONTUSION WITH A NEGATIVE X RAY. THE EMPLOYEE COMPLAINEDTHAT HIS SHOULDER WAS IN PAIN. AN MRI ON 3-13-03 REVEALED A MAJOR TEAR IN HIS ROTATOR CUFF.
WHILE EMPLOYEE WAS DESCENDING, HE STEPPED ON A PIPE HIS FOOT SLIPPED AND HIS RT KNEE HIT THE CONCRETE SURFACE.
EE WAS ASSISTING IN INSTALLING A RETURN ROLLER GUARD. WHILE HE WAS HOLDING THE GUARD IN PLACE, HIS FOOTING SLIPPED. RESULTED IN A STRAINED BACK. HAD RESTRICTED WORK DUTIES.
