Employee slipped on ice/snow. The resulting injury was treated as first aid initially. Employee informed that doctors are now recommending surgery. Surgery scheduled and employee set to return to work soon.

The employee was transferring from one rail car to the next when he slipped and fell landing on his shoulder. The employee reported pain in his ribs and shoulder. An MRI was taken on 07/17/15 that indicated a rotator cuff injury. Surgery is scheduled for 07/31/2015. The employee has been on light duty but it has not offset his normal job duties.

Doing daily pre-shift inspection on a Cat 777 haul truck. Employee tripped and lost his balance and fell. Tried to catch himself, landed on left hand and felt pain in left shoulder. Dr. restricted use of shoulder for one week and gave him a sling to wear

Employee was walking down stairs when he felt a pop and burning in his left knee around the front of his knee cap. He had sharp pain when walking, pain and swelling in his left knee with a dull ache while sitting. Physician found medial joint tenderness. Follow up exam indicated the need for surgery, performed 9/2/05.
