Showing all 6EE stepped off bottom rung of ladder; EE's pocket caught a pry bar which caused EE to fall. EE tried to stop fall with right arm while falling to ground. EE did not seek medical help or miss any time until recently. EE went to the doctor in July of 2019 and on 9/4/19 received an operation to repair a tear in EE's shoulder. Since the operation EE has been off work.
Employee was walking between cement pillars and tripped and fell. Employee put employee's arms out when falling and hurt employee's left hand/wrist.

EE set up a ladder to check a pipe. The ladder slipped and kicked out at the bottom. The EE fell down with the ladder, approx 15 feet. The EE sustained lacerations to his right arm and lower left leg.
