Employee was walking with a ladder in one arm and tools in the other. EE stepped on a rock and lost balance. EE fell on right shoulder and was taken to the ER for evaluation. EE was eventually diagnosed with an acute-on-chronic rotator cuff tear. EE has been out of work since that time because of the medical restrictions. Anticipated return date in late October.

Employee climbed up the ladder of bulk truck to open the wash out valve, while coming down EE twisted left ankle. Employee was diagnosed with a sprained ankle and was put on light duty with a walking boot restriction.
