While exiting the CAT 980 loader, employee was using 3 points of contact and facing the machine as required. During the descent, they believed they were on the last step and attempted to place a foot on the ground. However, they were actually on the second to last step and missed the final step, resulting in a fall to the ground that rolled their ankle.

Employee set up 8' step ladder under Auger on loam surface to clean out debris. Employee did not wait for fellow employee to secure the ladder and proceed to climb the ladder up to the third rung. The ladder shifted in the loam causing the ladder to fall and employee jumped to the ground. Ladder landed on employee's leg resulting in leg fracture.
Climbing loader laber, pulling self up into loader and felt a tear. Later in the day slipped when spraying water underneath one of the conveyors.
