Employee fell, while inside of a fall protection cage, on top of a pneumatic trailer during the loadout process. During the fall, the employee struck EE's right side breaking 3 ribs and received other non-reportable contusions and abrasions.

Showing all 10While climbing ladder to excavator, employee's foot slipped, resulting in discomfort in right calf. Injury did not require medical initially. Injury became reportable on 11/8/23 when employee reported swelling and bruising and injury became lost time.
Miner attempted to step on a short ladder leading to a conveyor service platform. Left foot slipped on ladder rung and right shin and knee hit the service platform mezzanine causing trauma to the knee cap. Miner did not have left foot in full contact with the ladder rung, only the tip of EE's boot. Ladder missing hand extension on the West side so did not have 3pt. contact.
Hatch on tanker trailer had bounced closed. Employee climbed up trailer to open hatch w/o fall protection and fell to ground. Employee incurred broken jaw and ribs, brain hemorrhage & contusions to liver, kidney & lung.
While walking on a slightly pitched roof, employee slipped and fell to the roof.
While stepping off a ramp onto a trailer top to work on a hatch, employee reports feeling left knee buckle.
We had a light snow / ice event that occurred early that morning that made the black top in the area slippery. He was on his way to punch in slipped on the black top and hurt his right leg. At this time doctor is indicating a pulled hamstring in the upper thigh area.
Employee climbed on top of a motor guard to check cleanliness of a Hammer Mill feeder pan. After checking, employee proceeded to climb down off of the guard and in doing so slipped and fell onto his back, striking either the guard or the concrete motor pad. Employee sustained fracture to the 8th rib on right side and sprain right ankle.
EE was walking on graveled area, slipped and reached for a oxy-acent torch cart to catch himself and struck his forearm on the hose holder, causing a cut to his right forearm.
HE WAS WORKING ON A DUST COLLECTOR WHEN HE SLIPPED OFF A LADDER AND TWISTED HIS LEFT ANKLE. HE WENT TO THE DOCTOR AND DR. SAID IT WAS A SLIGHT SPRAIN. REST IT FOR 24 TO 48 HOURS AND HE CAN COME BACK TO WORK WITH NO RESTRICTIONS.
