While climbing down from a ladder, miner grabbed an unsecure handle and was thrown off balance. Miner proceeded to jump off ladder and land on their feet due to off balance. Upon impact, miner fractured their right ankle. Height was approximately 6ft-9ft.

Showing all 10Miner parked a D9 Dozer and was climbing down to end shift. Miner's foot slipped and Miner was able to catch self on handrails. Miner had 3 points of contact during exit. Surface of step was slippery due to wet weather. Miner strained right shoulder, no lost work days. Miner is able to work Miner's normal duties with Miner's work restrictions.
Operator was doing pre-operation check when a snake jumped at him & he jumped back and fell.
EE was greasing a muddy loader when he lost his footing and fell off loader.

IT WAS RAINY, WET AND MUDDY. EE SLIPPED AND IMPACT WITH GROUND CASUED BACK TO LOCK UP.
HIS FOOT SLIPPED OFF LADDER WHILE HE WAS TRYING TO CLEAN THE REAR CAB WINDOW, CAUSING FOOT FIRST FALL TO GROUND.
EMPLOYEE STARTED CLIMBING LADDER. LADDER SHIFTED CAUSING HIM TO FALL BACKWARDS HITTING HIS RIGHT ELBOW.
