Showing all 7An over-the-road truck driver was on site to make customer pickup. Driver climbed up the ladder on the trailer to unroll the tarp, which was stuck in the partially unrolled position. Driver pulled on the tarp and it unrolled all at once, causing driver to lose balance and fall off the ladder. Driver fell to the ground and injured right foot/ankle upon impact with the ground.
Men were moving lengths of black poly pipe. They were using a forklift to pick up pipe off of ground to place on lowboy. A few pieces fell off lowboy. They went to retreive the pipe and almost had it back on lowboy when the weight of the pipe shifted and knocked him backwards and into a below ground level (6') containment area. He landed on his back.
EE was climbing out of excavator & slipped & lost his footing. His foot slipped off the step below the top of the track. His head hit the track.
Getting down in bed of truck - slipped - fell down & hit back of head.
WALKING DOWN A FLIGHT OF STAIRS, FOOT SLIPPED ON STEP, FELL TO THE BOTTOM. GOT UP AND SAT ON A PALLET TO CATCH A BREATH. NEXT THING HE REMEMBERSIS WAKING UP FACE FIRST ON THE GROUND, BACK WAS SCRATCHED, ELBOW CUT AND TEETH OUT OF MOUTH.
STEPPED OFF BANK OF SAND, SAND SLIP OUT FROM UNDERNEATH FEET, WITH ME, FELT ANKLE "POP".
