Contract employee was leaving the house of the dragline via the staircase. Once EE reached the bottom step, EE lost EE's balance and began to fall to the dragline shoe below (25 inches). EE's momentum carried EE forward, where EE fell off the dragline shoe to the ground below (51 inches). The impact to the ground resulted in 2 fractured vertebrae and lacerations to EE's face.

On 1-19-26, a Lube truck operator stumbled on dozer tracks and fell to the ground sustaining injury to their knee. Operator was able to work up until 2-7-26, when MRI results were received making this a lost time injury.

Employee fell on a seat arm rest on the shift change bus injuring their left rib area. Doctor diagnosed as first aid injury with no restrictions on 11-11-24. On 11-21-24 employee contacted safety indicating was still having discomfort. Employee seen by medical professional on 11-22-24 and placed on no work beginning on 11-23-24 until follow-up appointment on 12-3-24.

Employee was stepping over #2 hoist coupling to inspect #1 hoist motor and their foot slipped causing them to lose their balance. The employee reached out to catch themselves from falling hitting their left hand causing a dislocation of their left ring finger.

Employee was walking around the back side of a wheel pump and lost footing due to a washout. As the employee stumbled, their left foot went downward into the washout. As their right foot came down to keep them from falling, the right side of their knee came in contact with a piece of angle iron on the wheel pump causing laceration on right knee.

Mobile equipment operator of a 785D hauler fell through the access ladder approximately 4 1/2 to 5 feet from the platform to the ground while reaching to pick up their backpack and cooler off of the platform.

Employee had just finished parking and chocking the water truck in the equipment parking area and while walking toward the shop to get on another piece of equipment, the employee rolled their left ankle. Employee stated that the lighting was good in the area and that EE's boots were not the greatest.

Employee was getting inside of the shift change truck when their right knee gave out.

At the end of shift on 4-13-22, at approximately 4:40 am, a dozer operator that was working at the A2 area ramp was in the process of dismounting their dozer. While dismounting the dozer, the operator's foot slipped off the last step causing them to fall to the ground causing a strain / sprain to their left knee.

Employee was operating a farm tractor when the engine began to overheat, the employee lowered the loader and shut the tractor down. While opening the hood, the employee stepped over the loader tilt linkage when employee's foot slipped causing left knee to strike the loader tilt linkage resulting in a laceration about 3 inches below the knee requiring stitches to close the wound.

Employee stepped out of car in the employee parking lot prior to work shift and slipped on ice causing them to fall to the pavement. Employee reported soreness with hip, shoulder and bicep but did not request medical attention. Later on 4.8.21, employee started a series of physical therapy treatments which made this incident reportable to MSHA.

Employee was in the process of removing abandoned discharge line that was stuck in the mud, when EE lost balance and fell backwards (same level fall) striking the left side of face on the road causing dirt, sand to enter left eye. Employee taken to local clinic where eye was flushed and RX given for eye ointment for the irritation.

Employee was on the office porch. As ee walked to the door, ee fell injuring left ankle. The injury was evaluated by a doctor and initially diagnosed as a strain sprain. An MRI was performed to confirm and showed a fracture to the talus bone. The results from the MRI were reviewed with doctor on 12-13-2018 and resulted in classifying this injury as an MSHA Recordable.

Employee was dismounting an elevated platform using steps and handrails with 3 points of contact. One foot was on the ground, and when the second foot was going from the last step to the ground, the original foot slip out from under ee and caused them to fall.

Employee was pulling a 6" poly pipe line (for pumping water) with a nylon strap. The strap slipped (slack) and allowed employee to stumble backwards. Employee felt pain in lower back as a result. The pain got better but then worsened. On 11-8-17 employee was placed on restrictions which Luminant cannot accommodate. 11-9-17 was first lost time day.

Employee was coming down a set of steps just outside. As EE took last step with EE's right foot to the ground, EE heard and felt a pop just above the knee. EE fell to the right side on the ground. An ambulance was notified and employee was taken to Longview Regional Medical Center where the mechanic was diagnosed of having EE's right quadricep tendon ruptured.

A dozer operator was discovered lying beside a portable toilet in the field. EMS was contacted and the employee was transported to a local trauma center. It is believed that the employee passed out after exiting the toilet and fell back hitting head and back against the toilet's skid. Employee was diagnosed with two fractured vertebrae in lower back.

Electrician was pulling trail cable to put back on the cable shoe and slipped in the mud and fell face forward landing on hands and knees. The electrician felt pain in lower back. The electrician continued to work to get job done. The electrician decided to report the injury on 03/16/2017 as the pain worsened and was very sore. Rain and mud were a contributing factor.

The Dozer Operator was ascending back to the cab, lost footing striking right shin on the push arm of the tractor. The Operator did not know that the laceration had occurred until the Operator was back in the cab. The conditions were wet and muddy due to the rain event that was occurring.

While pulling on oil refill hoses the employee slipped on some oil on the PM bay floor. After slipping employee felt a twinge in knee. Employee was taken to the doctor on 1/30/17 and released full duty. On 3/1/17 employee was not able to return to work because of knee causing this to become recordable. Employee was scheduled to start physical therapy and see an Orthopedic surgeon.

Electrician was doing electrical maintenance repairs, when descending off a six foot ladder the electrician misplaced last step off of the ladder. Electrician lost balance and fell into a steel metal support in the revolving frame of the dragline, resulting in laceration to the arm requiring sutures.

Employee was dismounting from excavator. EE placed one foot on the ground and had one foot on the step when ee slipped and fell backwards, landing on buttock in the dirt. The impact caused a contusion to sacrum.

Employee was standing with one foot on a shelf and one foot on the top rung of a folded ladder apprx. 4' off the ground reaching up to remove an extension ladder being stored on top of a break room. When EE grabbed the ext. ladder, one section slid out causing the employee to lose balance and fall from the shelf and ladder upon which EE was standing on.

The employee was on a 992 loader near the front of the track and was making their way back to the platform when the employee slipped and fell to the ground.

Employee had completed fueling an excavator and was ascending wet and muddy wooden steps at the fuel station berm when EE slipped and fell striking EE's left shoulder. EE was returning the fuel nozzle to the tank.

The employee was pulling on a steam cleaner hose and tripped over dirt clod and fell back and strained employee's left hamstring.

Employee was using a steam cleaning unit to wash oil from the engine of a D9T Dozer. He was standing on the left dozer track when he slipped and fell, landing on the track, resulting in right shoulder pain. He worked full duty until his surgery on 1/28/2016.

Employee stepped into an uneven ground surface as he was dismounting his Excavator and sprained his left ankle.

Employee was walking across the hauler parking area, lost his footing and slipped in mud, causing a strain to the employees back.

A mechanic was performing maintenance on a Caterpillar 930G Loader in the shop. When was ascending the ladder to the cab he heard and felt a pop behind his left knee when he was on the last rung. Employee worked full duty until surgery was performed on 02/12/15.

Carrying a fire extinguisher up ladder and not using three points of contact while going down ladder. Resulting in fall and sustaining severe lower back injury.

During the Pre-Op inspection of 62008 Hauler the employee stepped on the side of a tire rut, rolled his right ankle twisting the right knee causing the employee to feel a pop in the knee

The injured was picking up rebar close to the base of a concrete pile when she slipped on a loose piece of concrete and lost her balance and fell, catching herself with her left hand causing a fracture to her left wrist.

Employee was attempting to mount bulldozer when his foot slipped, causing him to hit his head.

Employee was operating an End Dump. At the end of the shift he parked the end dump on the ready line north of the shop. As he was dismounting the machine he felt a pop in his right knee when he placed his right foot on the ground. The employee worked regular duty until June 26, 2014 when an MRI indicated a fractured bone at the end of the femur lopes on the upper part of the knee.

Employee was on hands and knees assisting in dump rope change out at dragline. Employee stated he was simply guiding rope through socket and stood up, turned to walk away, and felt pain in his lower left short rib area. On 05/23/2014 the injury became recordable due to restrictions.

On 4/16 employee dismounted forklift without facing the forklift or using three points of contact. Employee lost balance and twisted right knee when his boot caught the lip of the step. He said he did not need medical treatment and was retrained on proper mounting/dismounting procedure. On 5/13 he requested medical treatment for pain in right knee.

Employee was dismounting truck when grab iron broke. Employee fell backwards onto the ground. He injured his left shoulder, arm, and hip. Employee continued to work without restrictions or medical treatment. On 09-29-2014 he had rotator cuff surgery on his shoulder.

Employee was assigned to turn on grease lines at ALPM shop. He stepped up on four step roll around ladder and opened valve while descending the ladder he missed a step and fractured his right ankle.

Employee was assisting crew which was installing wooden transmission utility pole. Ground was frozen due to 19 degree temperature. When he attempted to roll the pole with a cant hook - his foot slipped on hard ground and he fell. He was taken to ER and X-ray determined that he fractured his fibula.

The employee was setting a pump when he twisted his left knee. He reported hearing a snap and feeling pain in the knee. He refused medical treatment at that time. On Jan 17, 2014, he asked to see a physician. He was diagnosed with a knee strain. After several doctor visits, his pain persisted. On Feb 21, 2014, physical therapy was prescribed, making this injury reportable.

Maintenance employee was washing the lube truck, when he finished washing one side of the truck he started walking to the other side and tripped on a piece of grating. The employee fell, but was able to catch himself with his hands before hitting the ground. He sustained a lumbar strain/sprain to his lower back from the fall.

Employee jumped into dragline swing case, shaft was removed, onto an OSB board in place to prevent tools from falling below. Board broke, employee fell into revolving frame, bruising his side.

Fuel mechanic was going to do a bucket and boom inspection on 36005 drag line. He was bending over to place his lock on the lock box that was located on the right shoe and his foot slipped due to mud on the shoe. He fell forward off the shoe about 7 feet to the ground fracturing his right wrist when hitting the ground.

The employee was using a 6' pry bar to adjust the brake rigging on a railcar. As the employee was pulling back on the bar it slipped out causing him to fall back and hit his head which resulted in a laceration that required sutures.

An electrician was going in the tub of a 2570 dragline to check the trial cable and strained his right Achilles tendon. On June 30, 2014 he was put in a soft case boot.

Employee was performing maintenance on a D9T Dozer. His wrench slipped as the nut released causing him to lose his balance. His momentum propelled him from the track bumping his tail bone and striking his left knee. He was taken to a medical clinic for treatment. Left knee was braced pending an MRI and evaluation. No climbing, squatting, working at elevation and ladders.

While walking on drag line catwalk, employee experienced a personal medical illness causing the employee to fall (same level) striking his head. A laceration was noticed on the back of the employee's head requiring 8 staples.

Dragline operator was dismounting dragline to go out and look at the tooth adapter on the bucket. As he was getting off the stop he rolled his right ankle fracturing the fibula bone just above the ankle. The operator will not be able to perform his work duties making this injury a lost time.

Employee was loading a D9T dozer on a haul truck trailer. After getting the dozer loaded and exiting the machine, he accidentally stepped off the outside edge of the trailer falling approximately 4 feet to the ground landing on his right hip. Employee was taken by ambulance to the hospital were he was diagnosed with a fractured right femur.
