Employee was getting in his vehicle at the end of his shift, while putting his foot on the running board, his foot slipped causing him to twist his other ankle. He went home and called his foreman to report the incident.

Showing all 22Employee was walking down a hill and slipped in mud, his left ankle twisted. He said he felt it pop and sharp pain. Employee was transported to HRMC ER at Prestonsburg Ky. X-rays were negative and was told to follow up with family Dr. His Family Dr. Examined him and took him off work till 8/30/11.
Employee was walking down the steps from the highwall miner launch and when he stepped off the last step onto the ground he stumbled and twisted his right ankle.
Employee was walking towards the underground forklift when he tripped on some mud, falling and twisting his right ankle.
EE walked behind the continuous miner to check his centerline. While walking back to his position to load another sc, he tripped over the miners waterline and fell on his left knee. Employee had completed shift and was crossing parking lot to get to his vehicle; this was his last day on the job. He fell and injured his left shoulder. On the way home employee stopped at ER. Examination indicated a dislocated left shoulder.
Leaving #7 entry go to left bolter, trip over rock, fall onto left knee. Complete shift, file injury report, go home, next day go to family Dr. has soft tissue damage & brusing. On 6-10-2010 find out employee not at work. Call him at home learn been taken off work until 6-14-2010. Employee not show up for work.
EE walking through intersection of #3 entry, trip over water line, and fell onto SCSR injured low back area. Taken to ER, x-ray negative, released to RTW 9/2/09. On 9/2/09 saw Dr. at Family Health Center, taken off work..
Employee was inspecting bearing on crusher. When getting down from crusher, the ladder fell approx 4 feet, landing against plant wall. Employee saw doctor, but continued to work. M.D. referred him to Neuro. Last day worked 12-30-09.
Employee was exiting Cat. 990-G end loader. When he stepped onto the ground with his right foot, his left foot got hung on the step causing him to fall to the ground; resulting in injury to the left knee.

EE AND ANOTHER EE AT SURFACE SHOP PICKING UP A PARTS SHELF WHILE DOING, INJURED TRIPPED OVER A CRIB BLOCK AND THE RESULTING MOTION CAUSED A SPRAIN TO LEFT KNEE. ON 11-27-01 INJURED SAW A DR. AN ORTHOPEDIC SURGEON, WHO PLACED HIM ON RESTRICTED DUTY TILL DEC.4TH, INJURED RETURNED TO WORK ON 11-27-01, DOING LIGHT WORK IN HIS REGULAR JOB.
EMPLOYEE WAS STANDING ON A SCOOP WHEN HE SLIPPEDCAUSING HIS WEIGHT TO SHIFT TO HIS RIGHT SIDE. THIS CAUSED PAIN AND SWELLING IN HIS RIGHT KNEE THAT HAD BEEN PREVIOUSLY INJURED ON 11-10-99.
EMPLOYEE WAS IN THE PROCESS OF MOUNTING THE OPERATOR'S CAB OF GREASE TRUCK. HE PLACED HIS LEFT FOOT ON A STEP AND HIS RIGHT HAND ON A GRAB RAIL AND AS HE BEGAN HIS CLIMB HIS LEFT FOOT SLIPPED AND HE RETURNED TO GROUND LEVEL. CAUSED A BURNING SENSATION IN HIS LEFT KNEE.
EMPLOYEE WAS STEPPING OVER A PADDLE OF WATER WHEN HE EXPERIENCED PAIN IN HIS BACK.
EE BEGAN TO WALK FROM THE CENTER OF AN ENTRY TO THE RIGHT SIDE WHEN HE STATED THAT HIS LEFT KNEE GAVE WAY. EE HAS HAD SURGERY THREE TIMES ONT HIS SAME EDGE DATING BACK TO 1985. WORKPLACE EXPOS URE ON THIS DATE WAS NOT A CONTIBUTING FACTS.
EE WAS DESCENDING FROM COAL LOADER WHEN HE STATED HE STEPPED ON A LUMP OF COAL ON THE STEP RESULTING IN A FALL.
