I.NAME WAS INJURED ON 6-12-84 WHEN COMING BACK TO WORK. HE HAD WALKED AWAY FROM THE WORK AREA DURING LUNCH & WAS HEADED BACK TO THE WORK AREA. WHEN HE THOUGHT HE WOULD TAKE A SHORT-CUT DOWN A 10\ SLIGHTLY INCLINED HIGHWALL. WHILE CLIMBING DOWN THIS HIGHWALL HE SLIPPED, FELL, STRUCK THE GORUND & ROLLED ABOUT 30\ CAUSING SMALL CUT ABRASION & A SPRAINED ANKLE.

SUBJECT WAS INJURED WHILE WALKING BACK TO THE REAR OF THE GREASE TRUCK HE WAS FILLING WITH FUEL WHILE WALKING BACK HE SLIPPED AND FELL OFF THE TRUCK HITTING THE GROUND ON HIS LOWER PART OF BAC K AND RIGHT PELVIS AREA RESULTING IN DOUBLE FRACTURE OF UPPER FEMUR BONE
